摘 要:目的探讨非小细胞肺癌(NSCLC)患者放疗期间放射性心脏损伤(RIHD)的影响因素。方法回顾性分析2020年2月至2021年2月本院收治的60例NSCLC患者临床资料,按照是否发生RIHD,分为发生患者(n=19)与未发生患者(n=41),收集两类患者的临床资料[年龄、性别、肿瘤分类、高血压、糖尿病、病理分型、分割方式、转化生长因子-β_(1)(TGF-β_(1))水平],采用多因素Logistic回归分析NSCLC患者放疗期间RIHD的影响因素。结果两类患者年龄、性别、肿瘤分型、有无糖尿病占比、病理分型比较差异无统计学意义;发生患者有高血压占比、常规分割占比、TGF-β_(1)水平均高于未发生患者,差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,合并高血压、常规分割方式、TGF-β_(1)高表达是NSCLC患者放疗期间RIHD发生的影响因素(P<0.05)。结论NSCLC患者放疗期间RIHD发生受合并高血压、常规分割方式、TGF-β_(1)高表达影响,临床可根据患者存在的上述风险因素,进行针对性干预,以降低RIHD风险。Objective To investigate the influential factors of radiation induced heard disease(RIHD)during radiotherapy in patients with non-small cell lung cancer(NSCLC).Methods The clinical data of 60 patients with NSCLC admitted to our hospital from February 2020 to February 2021 were retrospectively analyzed,according to whether RIHD occurred,they were divided into patients with RIHD(n=19)and patients without RIHD(n=41).The clinical data(age,sex,tumor classification,hypertension,diabetes,pathological classification,segmentation,transforming growth factor-β_(1)[TGF-β_(1)]level)of the two types of patients were collected,and multivariate Logistic regression analysis were used to analyze the influencing factors of RIHD during radiotherapy in NSCLC patients.Results There were no significant differences in age,sex,tumor type,diabetes or pathological type between the two types of patients.The proportion of patients with hypertension,the proportion of routine segmentation and the level of TGF-β_(1) were higher than those without hypertension,and the differences were statistically significant(P<0.05).Multiple Logistic regression analysis showed that hypertension,conventional segmentation and TGF-β_(1) high expression were the factors affecting the occurrence of RIHD in NSCLC patients during radiotherapy(P<0.05).Conclusion The occurrence of RIHD in patients with NSCLC during radiotherapy is affected by hypertension,conventional segmentation,and high expression of TGF-β_(1),targeted interventions can be carried out according to the above risk factors in patients to reduce the risk of RIHD.
