摘　　要：目的分析尿毒症血液透析患者低蛋白血症发生状况及其影响因素。方法回顾性分析2018年5月至2020年2月于本院接受血液透析治疗的100例尿毒症患者的临床资料,评估尿毒症血液透析患者低蛋白血症发生情况,统计患者临床资料,采用多因素Logistic回归分析尿毒症患者血液透析后低蛋白血症发生的影响因素。结果100例尿毒症血液透析患者中,发生低蛋白血症36例,占比36.00%。发生低蛋白血症与未发生低蛋白血症患者年龄、性别、有无吸烟史、血肌酐(SCr)水平比较差异无统计学意义;发生低蛋白血症患者营养不良占比高于未发生低蛋白血症患者,透析时间长于未发生低蛋白血症患者,超敏C反应蛋白(hs-CRP)水平高于未发生低蛋白血症患者,差异有统计学意义(P<0.05)。Logistic回归分析结果显示,营养不良、透析时间较长、hs-CRP水平较高均为尿毒症血液透析患者低蛋白血症发生的影响因素(OR>1,P<0.05)。结论尿毒症血液透析患者低蛋白血症发生率较高,且营养不良、透析时间较长、hs-CRP水平较高均为尿毒症血液透析患者低蛋白血症发生的影响因素,临床可据此实行针对性处理措施,防治低蛋白血症。Objective To analyze the occurrence and influencing factors of hypoproteinemia in hemodialysis patients with uremia.Methods The clinical data of 100 patients with uremia who received hemodialysis treatment in our hospital from May 2018 to February 2020 were retrospectively analyzed,and the occurrence of hypoproteinemia in patients with uremia who received hemodialysis was evaluated.The clinical data of the patients were statistically analyzed,and multivariate Logistic regression analysis were used to analyze the influencing factors that might lead to the occurrence of hypoproteinemia in patients with uremia after hemodialysis.Results Among 100 uremic hemodialysis patients,36 cases(36.00%)had hypoproteinemia.There was no significant difference in age,gender,smoking history and serum creatinine(SCr)level between patients with hypoproteinemia and without hypoproteinemia.The proportion of malnutrition in patients with hypoproteinemia was higher than that in patients without hypoproteinemia,the dialysis time was longer than that in patients without hypoproteinemia,and the level of hypersensitive C reactive protein(hs-CRP)was higher than that in patients without hypoproteinemia,the difference was statistically significant(P<0.05).Multivariate Logistic regression analysis showed that malnutrition,long dialysis time and high hs-CRP level were all factors affecting the occurrence of hypoproteinemia in hemodialysis patients with uremia(OR>1,P<0.05).Conclusions The incidence of hypoproteinemia in hemodialysis patients with uremia is high,and malnutrition,long dialysis time and high hs-CRP level are all factors affecting the occurrence of hypoproteinemia in hemodialysis patients with uremia.Therefore,targeted treatment measures can be implemented to prevent and treat hypoproteinemia.
