摘 要:目的探究急性左心衰竭合并呼吸衰竭早期应用无创机械通气的临床疗效。方法选取2019年2月至2021年2月于本院接受治疗的122例急性左心衰竭合并呼吸衰竭早期患者作为研究对象,随机分为对照组与观察组,每组61例。对照组予以面罩吸氧治疗,观察组则实施无创机械通气治疗,比较两组预后转归指标及治疗后的血气指标。结果观察组病情好转时间短于对照组,好转率高于对照组,气管插管率及病死率均低于对照组,差异有统计学意义(P<0.05)。观察组呼吸频率(RR)、动脉血二氧化碳分压(PaCO_(2))均显著低于对照组,血氧饱和度(SPO_(2))、动脉血氧分压(PaO_(2))均明显高于对照组,差异有统计学意义(P<0.05)。结论在急性左心衰竭合并呼吸衰竭早期应用无创机械通气治疗的效果显著,能有效改善患者的血气指标,提高病情好转率,降低病死率,具有较高的临床应用价值。Objective To explore the clinical efficacy of noninvasive mechanical ventilation in the early stage of acute left heart failure complicated with respiratory failure.Methods A total of 122 patients with acute left heart failure combined with early respiratory failure who received treatment in our hospital from February 2019 to February 2021 were selected as the study subjects,they were randomly divided into the control group and the observation group,with 61 cases in each group.The control group was given oxygen inhalation by mask,and the observation group was given no invasive mechanical ventilation,the prognostic indexes and blood gas indexes after treatment were compared between the two groups.Results The improvement time in the observation group was significantly shorter than that in the control group,and the improvement rate was significantly higher than that in the control group,tracheal intubation rate and mortality rate were significantly lower than control group,the differences were statistically significant(P<0.05).Respiratory rate(RR)and partial pressure of arterial blood carbon dioxide(PaCO_(2))in the observation group were significantly lower than those in control group,and oxygen saturation(SPO_(2))and partial pressure of arterial blood oxygen(PaO_(2))in the observation group were significantly higher than those in the control group,the difference was statistically significant(P<0.05).Conclusion Noninvasive mechanical ventilation is effective in the early treatment of acute left heart failure combined with respiratory failure,which can effectively improve the blood gas index of patients,increase the disease recovery rate and reduce the mortality rate,and has high clinical application value.
