摘　　要：目的探讨子宫内膜容受性及动脉血流动力学参数检测在不孕症患者中的应用价值。方法选取2020年10月至2021年3月本院妇产科就诊的48例不孕症患者作为观察组,另选取同期备孕定期检查的48名健康育龄女性作为对照组。两组均进行子宫超声检查,比较两组子宫内膜状况、子宫动脉血流动力学参数[收缩期峰值血流速度(PSV)、博动指数(PI)、阻力指数(RI)]、雌激素[雌二醇(E2)、孕酮(P)]水平,采用ROC曲线评价子宫内膜厚度、子宫内膜容积及子宫动脉血流动力学参数对不孕症的诊断价值。结果观察组子宫内膜厚度薄于对照组,子宫内膜容积小于对照组,差异有统计学意义(P<0.05);观察组A型子宫内膜占比低于对照组,B型、C型子宫内膜占比均高于对照组,差异有统计学意义(P<0.05)。观察组PI、RI均大于对照组,PSV快于对照组,差异有统计学意义(P<0.05)。观察组E2水平高于对照组,P水平低于对照组,差异有统计学意义(P<0.05)。ROC曲线结果显示,子宫内膜容积诊断效能优于子宫内膜厚度,PI、RI对不孕症的诊断效能良好,PSV的诊断效能较弱。结论子宫内膜厚度、子宫内膜容积及子宫动脉血流动力学参数均是影响临床妊娠的重要因素,对诊断不孕症具有参考价值。Objective To investigate the application value of endometrial receptivity and uterine arterial hemodynamic parameters in infertility patients.Methods 48 infertility patients from the Obstetrics and Gynecology department of our hospital from October 2020 to March 2021 were selected as the observation group,and 48 healthy women of childbearing age who were preparing for regular check-ups during the same period were selected as the control group.Uterine ultrasound was performed in two groups,the endometrial status,uterine arterial hemodynamic parameters(peak systolic velocity[PSV],pulsatility index[PI],resistance index[RI])and estrogens(estradiol[E2],progesterone[P])were compared between the two groups,the ROC curve was used to evaluate the diagnostic value of endometrial thickness,endometrial volume and uterine arterial hemodynamic parameters for infertility.Results The endometrial thickness in the observation group was thinner than that in the control group,and the endometrial volume was smaller than that of the control group,and the differences were statistically significant(P<0.05);the proportion of type A endometrium in the observation group was lower than that in the control group,and the proportion of type B and type C endometrium were higher than those in the control group,and the differences were statistically significant(P<0.05).The PI and RI in the observation group were higher than those in the control group,and the PSV was faster than that in the control group,and the differences were statistically significant(P<0.05).The E2 level in the observation group was higher than that in the control group,and the P level was lower than that in the control group,and the differences were statistically significant(P<0.05).ROC curve results showed that endometrial volume had better diagnostic efficacy than endometrial thickness,PI and RI had better diagnostic efficacy for infertility,and PSV had weaker diagnostic efficacy.Conclusion Endometrial thickness,endometrial volume and uterine arterial hemodynamic parameters
