关节镜下缝合修补术和半月板次全/全切除术治疗半月板桶柄样撕裂的临床疗效比较

摘　　要：目的探讨关节镜下缝合修补术和半月板次全/全切除术治疗半月板桶柄样撕裂的临床疗效。方法选取2019年1月至2020年3月本院收治的64例半月板桶柄样撕裂患者作为研究对象,根据手术方案不同分为半月板组(n=30)与关节镜组(n=34)。关节镜组采取关节镜下缝合修补术治疗,半月板组采取半月板次全/全切除术治疗,比较两组膝关节功能、膝关节活动度、围手术期指标(手术时间、住院时间)、临床疗效及并发症发生情况。结果术前,两组Lysholm评分和膝关节活动度比较差异无统计学意义;末次随访,关节镜组Lysholm评分、膝关节活动度均高于半月板组,差异有统计学意义(P<0.05)。关节镜组手术时间长于半月板组,住院时间短于半月板组,差异有统计学意义(P<0.05)。两组治疗总有效率比较差异无统计学意义。两组并发症发生率比较差异无统计学意义。结论关节镜下行缝合修补术和半月板次全/全切除术治疗半月板桶柄样撕裂疗效相当,但关节镜下行缝合修补术能有效改善膝关节功能及活动度,缩短住院时间,临床可根据患者病情和需求进行合理选择。Objective To investigate the comparison of clinical efficacy of arthroscopic suture repair and subtotal/total meniscectomy in the treatment of barrel handle tear meniscus.Methods 64 barrel handle tear meniscus patients admitted to our hospital from January 2019 to March 2020 were selected as the study subjects,and they were divided into the meniscus group(n=30)and the arthroscopic group(n=34)according to different surgical protocols.The arthroscopic group was treated with arthroscopic suture repair,and the meniscus group was treated with subtotal/total meniscectomy,the knee joint function,knee joint range of motion,perioperative indicators(operation time,hospitalization time),clinical efficacy and complications were compared between the two groups.Results Before operation,there was no significant difference in Lysholm score and knee range of motion between the two group;at last follow-up,Lysholm score and knee motion in the arthroscopic group were higher than those in the meniscus group,and the differences were statistically significant(P<0.05).The operation time in the arthroscopic group was longer than that in the meniscus group,and the hospital stay was shorter than that in the meniscus group,the differences were statistically significant(P<0.05).There was no significant difference in the total effective rate between the two groups.There was no significant difference in the complication rate between the two groups.Conclusion Arthroscopic suture repair has the same efficacy as subtotal/total meniscectomy in the treatment of barrel handle tear meniscus,but suture repair under arthroscopy can effectively improve the knee joint function and range of motion,shorten the and hospitalization time,and can be selected according to the patient's condition and needs.
