锁骨远端锁定钢板联合克氏针张力带技术治疗锁骨远端NeerⅡ型骨折的应用价值

摘　　要：目的探讨锁骨远端锁定钢板联合克氏针张力带技术治疗锁骨远端NeerⅡ型骨折的应用价值。方法选取2019年3月至2021年8月本院收治的80例锁骨远端NeerⅡ型骨折患者作为研究对象,随机分为常规组与研究组,各40例,常规组给予克氏针张力带技术治疗,研究组给予锁骨远端锁定钢板联合克氏针张力带技术治疗,比较两组治疗优良率、术后并发症发生率、肩关节功能评分(Constant-Murley评分)、视觉模拟评分法(VAS)评分。结果研究组治疗优良率为100.00%,高于常规组的80.00%,差异有统计学意义(P<0.05)。研究组并发症发生率为2.50%,低于常规组的30.00%,差异有统计学意义(P<0.05)。研究组功能、症状、肌力、活动度评分及总分均高于常规组,差异有统计学意义(P<0.05)。治疗前,两组VAS评分比较差异无统计学意义;治疗后,两组VAS评分均低于治疗前,且研究组低于常规组,差异有统计学意义(P<0.05)。结论锁骨远端锁定钢板联合克氏针张力带技术治疗锁骨远端NeerⅡ型骨折疗效确切,可明显降低活动受限、疼痛合并异物感、针尾皮肤感染、内固定松动等并发症发生率,增强肩关节功能恢复效果,缓解疼痛程度,提高患者总体预后水平。Objective To investigate the application value of distal clavicular locking plate combined with Kirschner wire tension band technique in the treatment of distal clavicular Neer typeⅡfracture.Methods A total of 80 patients with distal clavicle Neer typeⅡfracture admitted to our hospital from March 2019 to August 2021 were selected as the research subjects,and they were randomly divided into the conventional group and the study group,with 40 cases in each group.The conventional group was treated with Kirschner wire tension band technique,while the research group was treated with distal clavicular locking plate combined with Kirschner wire tension band technique,the excellent and good rate of treatment,incidence of postoperative complications,shoulder joint function score(Constant-Murley score)and visual analogue scale(VAS)score were compared between the two groups.Results The rate of good and good treatment in the study group was 100.00%,which was higher than 80.00%in the conventional group,and the difference was statistically significant(P<0.05).The incidence of complications in the study group was 2.50%,which was lower than 30.00%in the conventional group,and the difference was statistically significant(P<0.05).The scores of function,symptoms,muscle strength,range of motion and total scores in the study group were higher than those in the conventional group,and the differences were statistically significant(P<0.05).Before treatment,there was no significant difference in VAS scores between the two groups;after treatment,VAS scores of two groups were lower than before treatment,and the study group was lower than the conventional group,the difference was statistically significant(P<0.05).Conclusion Distal clavicular locking plate combined with Kirschner wire tension band technique is effective in the treatment of Neer typeⅡfracture distal clavicle,which can significantly reduce the incidence of complications such as limited mobility,pain combined with foreign body sensation,pintail skin infection,and internal
