摘 要:目的探究体外冲击波联合玻璃酸钠对早中期膝骨性关节炎(KOA)患者关节液生物标记物的影响。方法选取2020年12月至2021年10月九江市中医医院收治的64例早中期KOA患者作为研究对象,按照随机数字表法分为对照组与实验组,各32例。对照组行膝关节腔内注射玻璃酸钠治疗,实验组在对照组基础上联合体外冲击波治疗,比较两组关节液生物标记物[白细胞介素-17(IL-17)、趋化因子12(CXCL12)、基质金属蛋白酶-1(MMP-1)]水平、西安大略和麦克马斯特大学(WOMAC)骨关节炎指数、临床疗效及不良反应发生情况。结果治疗前,两组IL-17、CXCL12、MMP-1水平及WOMAC骨关节炎指数比较差异无统计学意义;治疗后,两组IL-17、CXCL12、MMP-1水平及WOMAC骨关节炎指数均低于治疗前,且实验组低于对照组,差异有统计学意义(P<0.05)。实验组治疗总有效率为93.75%,高于对照组的75.00%,差异有统计学意义(P<0.05)。两组治疗后均未发生不良反应。结论体外冲击波联合玻璃酸钠治疗早中期KOA患者效果显著,可明显改善关节液生物标记物水平,且无明显不良反应,值得临床推广应用。Objective To explore the effect of extracorporeal shock wave combined with sodium hyaluronate on the biomarkers of joint fluid in patients with early and middle knee osteoarthritis.Methods 64 patients with early and middleknee osteoarthritis treated in Jiujiang Traditional Chinese Medicine Hospital from December 2020 to October 2021 were selected as the research subjects,and they were divided into control group and experimental group,with 32 cases in each group.The control group was treated with intra-articular injection of sodium hyaluronate,and the experimental group was treated with extracorporeal shock wave on the basis of the control group,the levels of joint fluid biomarkers(interleukin-17[IL-17],chemokine 12[CXCL12],matrix metalloproteinase-1[MMP-1]),Western Ontario and Master Universitiees(WOMAC)osteoarthritis index,clinical efficacy and adverse reactions were compared between the two groups.Results Before treatment,there were no significant differences in the levels of IL-17,CXCL12,MMP-1 and WOMAC osteoarthritis index between the two groups;after treatment,the levels of IL-17,CXCL12,MMP-1 and WOMAC osteoarthritis index of the two groups were lower than those before treatment,and the experimental group was lower than the control group,the differences were statistically significant(P<0.05).The total effective rate of the experimental group was 93.75%,which was higher than 75.00%of the control group,the difference was statistically significant(P<0.05).No adverse reactions occurred in the two groups after treatment.Conclusion The extracorporeal shock wave combined with sodium hyaluronate in the treatment of early and middle KOA patients has significant effect,which can significantly improve the level of joint fluid biomarkers without obvious adverse reactions,which is worthy of clinical promotion and application.
