瓜蒌贝母散合二陈汤治疗老年性肺炎的临床疗效

摘　　要：目的探讨瓜蒌贝母散合二陈汤治疗老年性肺炎的临床疗效。方法选取2019年10月至2020年10月本院收治的86例老年性肺炎患者作为研究对象,按治疗方法不同分为实验组(n=40)与对照组(n=46)。对照组采取常规治疗,实验组在对照组基础上采用瓜蒌贝母散合二陈汤治疗,比较两组临床疗效、炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、C反应蛋白(CRP)]水平、免疫细胞(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))水平、体液免疫(IgA、IgG、IgM)水平及预后情况。结果实验组治疗总有效率为92.50%,明显高于对照组的76.09%,差异有统计学意义(P<0.05)。治疗后,两组TNF-α、IL-6、CRP水平均低于治疗前,且实验组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)均高于治疗前,CD8^(+)低于治疗前,且实验组CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)均高于对照组,CD8^(+)低于对照组,差异有统计学意义(P<0.05)。治疗后,两组IgA、IgG、IgM水平均高于治疗前,且实验组高于对照组,差异有统计学意义(P<0.05)。实验组咳嗽、咳痰、发热持续时间及住院时间均短于对照组,差异有统计学意义(P<0.05)。结论瓜蒌贝母散合二陈汤治疗老年性肺炎疗效显著,可有效降低患者炎症反应,提高免疫功能,加快患者机体恢复,值得临床推广应用。Objective To investigate the clinical efficacy of Gualou Beimu powder combined with Erchen decoction in the treatment of senile pneumonia.Methods A total of 86 patients with senile pneumonia who were treated in the hospital from October 2019 to October 2020 were selected as the study subjects,they were divided into the experimental group(n=40)and the control group(n=46)according to different treatment methods.The control group received conventional treatment,while the experimental group received Gualou Beimu powder combined with Erchen decoction on the basis of the control group,the clinical efficacy,inflammatory factors(tumor necrosis factor-α[TNF-α],interleukin-6[IL-6]and C-reactive protein[CRP])level,cellular immunity(CD3^(+),CD4^(+),CD8^(+)and CD4^(+)/CD8^(+))level,humoral immunity(IgA,IgG,IgM)level and the prognosis were compared between the two groups.Results The total effective rate in the treatment group was 92.50%,which was significantly higher than 76.09%in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of TNF-α,IL-6 and CRP of the two groups were lower than before treatment,and the experimental group was lower than the control group,the differences were statistically significant(P<0.05).After treatment,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)of the two groups were higher than before treatment,and the level of CD8^(+)was lower than before treatment,and the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the experimental group were higher than the control group,and the level of CD8^(+)was lower than the control group,the differences were statistically significant(P<0.05).After treatment,the levels of IgA,IgG and IgM of the two groups were higher than before treatment,and the experimental group was higher than the control group,the differences were statistically significant(P<0.05).The duration of cough,sputum,fever and hospitalization time in the experimental group were shorter than those in the control group,and the differences were statis
