高龄孕妇妊娠晚期的异常心电图情况与妊娠结局的关系

摘　　要：目的探讨高龄孕妇妊娠晚期的异常心电图情况与妊娠结局的关系。方法选取2019年1月至2020年4月本院80名妊娠晚期孕妇作为研究对象,根据孕妇年龄不同分为高龄组(≥35岁,n=42)、适龄组(20~34岁,n=38)。孕妇均行心电图检查,比较两组异常心电图、心律失常亚型及妊娠结局情况。结果高龄组心律失常、电轴偏左、P-R间期缩短、导联低电压、ST-T段压低发生率分别为33.33%、19.05%、21.43%、19.05%、16.67%,均高于适龄组的13.16%、2.63%、2.63%、2.63%、0.00%,差异有统计学意义(P<0.05)。高龄组窦性心动过速发生率为19.05%,高于适龄组的2.63%,差异有统计学意义(P<0.05);两组室性心律不齐、窦性心动过缓、室内传导阻滞、房性早搏、室性早搏发生率比较差异无统计学意义。心电图正常孕妇中,高龄组早产率为41.94%,高于适龄组的6.06%,顺产率为45.16%,低于适龄组的84.85%,差异有统计学意义(P<0.05);心电图异常孕妇中,高龄组顺产率为18.18%,低于适龄组的80.00%,差异有统计学意义(P<0.05)。结论高龄孕妇妊娠晚期易出现心律失常、电轴偏左、P-R间期缩短、导联低电压、ST-T段压低、窦性心动过速等情况,且异常心电图妊娠晚期高龄孕妇的妊娠结局较差,因此,临床需加强对妊娠晚期高龄孕妇的心电图监护,并及时予以针对性干预,以改善妊娠结局,确保母婴安全。objective To explore the relationship between abnormal electrocardiogram and pregnancy outcome in elderly pregnant women in the third trimester.Methods 80 elderly pregnant women in the third trimester of our hospital from January 2019 to April 2020 were selected as the research subjects,and they were divided into the elderly group(≥35 years old,n=42)and the age-appropriate group(20-34 years old,n=38)according to different ages of pregnant women.All pregnant women underwent electrocardiogram examination,and the abnormal electrocardiogram,arrhythmia subtypes and pregnancy outcome were compared between the two groups.Results The incidence of arrhythmia,left axis deviation,shortened PR interval,low lead voltage and ST-T segment depression in the elderly group were 33.33%,19.05%,21.43%,19.05%and 16.67%,respectively,which were higher than 13.16%,2.63%,2.63%,2.63%,0.00%in the age-appropriate group,and the differences were statistically significant(P<0.05).The incidence of sinus tachycardia in the elderly group was 19.05%,which was higher than 2.63%in the age-appropriate group,and the difference was statistically significant(P<0.05).There were no significant differences in the incidence of smothering arrhythmia,sinus bradycardia,indoor conduction block,atrial premature beat and smothering premature beat between the two groups.Among the pregnant women with normal electrocardiogram,the premature delivery rate in the elderly group was 41.94%,which was higher than 6.06%in the age-appropriate group,the natural delivery rate was 45.16%,which was lower than 84.85%in the age-appropriate group,and the differences were statistically significant(P<0.05).Among the pregnant women with abnormal electrocardiogram,the rate of natural delivery in the elderly group was 18.18%,which was lower than 80.00%in the age-appropriate group,and the difference was statistically significant(P<0.05).Conclusion Elderly pregnant women in the third trimester are prone to arrhythmia,left axis deviation,shortened P-R interval,low lead voltage,ST-T segmen
