摘 要:目的分析超声评估主动脉缩窄胎儿主动脉弓狭窄部位的血流动力学特点。方法选取2019年5月至2020年5月本院收治的30例胎儿确诊主动脉缩窄(COA)孕妇作为观察组,另选取同期产检结果正常的30名孕妇作为对照组。比较两组不同孕期收缩期峰值血流速度(SPv)、舒张末期血流速度(EDv)、搏动指数(PI)。结果观察组不同孕期SPv、EDv、PI比较差异无统计学意义;对照组不同孕期SPv、PI比较差异有统计学意义(P<0.05),不同孕期EDv比较差异无统计学意义。组间比较:孕24~28、>28~32周,观察组SPv、EDv均快于对照组,PI均低于对照组,差异有统计学意义(P<0.05);>32~36、>36~38周,两组SPv、EDv、PI比较差异无统计学意义。组内比较:观察组SPv、EDv、PI不同孕期两两比较差异无统计学意义;对照组>32~36周孕妇SPv、EDv均快于24~28周,>36~38周孕妇SPv快于24~28周,PI高于24~28周,>36~38周孕妇PI均高于>28~32周与>32~36周,差异有统计学意义(P<0.05),其余孕龄两两比较差异无统计学意义。结论不同孕龄的COA胎儿主动脉弓部狭窄处的血流状态具有不同表现,对其进行超声评估时,还应考虑孕妇孕龄情况。Objective To analyze the ultrasound evaluation of the hemodynamic characteristics of aortic arch stenosis in fetus with coarctation of aorta.Methods 30 pregnant women whose fetuses were diagnosed with coarctation of aorta(COA)in our hospital were selected as the observation group,in addition,30 pregnant women with normal prenatal examination results during the same period were selected as the control group.The peak systolic velocity(SPv),end diastolic blood flow velocity(EDv),and pulsatile index(PI)were compared between the two groups during different pregnancy.Results There were no significant differences in SPv,EDv and PI in the observation group during different pregnancies periods;there were significant differences in SPv and PI in the control group during different pregnancies(P<0.05),but no significant differences in EDv during different pregnancies.Comparison between groups:at 24-28 weeks and>28-32 weeks of pregnancy,SPv and EDv in the observation group were faster than those in the control group,and PI was lower than that in the control group,the differences were statistically significant(P<0.05);there was no significant difference in SPv,EDv and PI between the two groups at>32-36 weeks and>36-38 weeks.Comparison within the group:there was no significant difference in SPv,EDv and PI in the observation group during different pregnancies;in the control group,SPv and EDv of pregnant women>32-36 weeks were faster than 24-28 weeks,SPv of pregnant women>36-38 weeks was faster than 24-28 weeks,PI was higher than 24-28 weeks,PI of pregnant women>36-38 weeks was higher than>28-32 weeks and>32-36 weeks,and the differences were statistically significant(P<0.05).Conclusion COA fetuses of different gestational weeks have different manifestations of blood flow at the stenosis of the aortic arch,the gestational age should also be considered when performing ultrasound evaluation.
