摘　　要：目的探讨MRI在强直性脊柱炎(AS)诊断中的应用价值。方法选取2019年1月至2020年12月本院收治的110例疑似AS脊柱病变患者作为研究对象,均进行CT、MRI检查。以手术确诊为金标准,比较两种检查方式的灵敏度、特异度及符合率。结果110例疑似AS脊柱病变患者中,经手术确诊87例,确诊率为79.09%;MRI检查的诊断灵敏度、符合率、特异度分别为97.73%、97.27%、95.45%,均高于CT检查的86.42%、74.55%、41.38%,差异有统计学意义(P<0.05)。结论与CT检查比较,MRI检查对AS脊柱病变患者的诊断价值较高,具有较高诊断灵敏度、符合率、特异度,且检查方式操作简便、安全,可作为诊断AS脊柱病变的首选方法。Objective To investigate the diagnostic value of MRI in ankylosing spondylitis(AS).Methods 110 patients with suspected as AS admitted to our hospital from January 2019 to December 2020 were selected as the research subjects.All patients were examined by CT and MRI,the sensitivity,specificity and coincidence rate of the two methods were compared with the gold standard of surgical diagnosis.Results Among the 110 suspected patients with AS spinal lesions,87 cases were confirmed by operation,the confirmed rate was 79.09%.The diagnostic sensitivity,coincidence rate and specificity of MRI were 97.73%,97.27%and 95.45%,respectively,which were higher than 86.42%,74.55%and 41.38%of CT examination,and the differences were statistically significant(P<0.05).Conclusion Compared with CT examination,MRI examination has higher diagnostic value for patients with AS spinal lesions,which has high diagnostic sensitivity,coincidence rate,specificity,and the examination method is easy and safe to operate,and it may be the first choice for diagnosis of AS spinal lesions.
