父亲生育年龄与子代孕龄及早产和足月早期产率的关系

摘　　要：目的探究父亲生育年龄与新生儿孕龄及早产和足月早期产率的关系。方法以北京市通州区孕期保健系统和通州区妇幼保健院阴道自然分娩的13548例单胎活产新生儿及其父母亲的医院信息系统为基础进行回顾性队列研究。结果新生儿平均出生孕龄为(39.0±1.4)周,其中完全足月儿(39~40^(+6)周)占61.7%,早产儿(<37周)占4.0%,足月早期产儿(37~38^(+6)周)占26.1%。父亲平均生育年龄为(30.0±4.5)岁。与父亲生育年龄20~25岁组相比,父亲生育年龄较大的各组新生儿早产率均较高,各组的调整后比数比(a OR)在1.74至2.45之间(95%置信区间均大于1),多因素趋势分析a OR=1.11(95%CI,1.06~1.15)。未见父亲生育年龄大于25岁组与足月早期产率的关联有统计学显著性意义。结论父亲生育年龄大于25岁可能是子代早产的潜在危险因素,还需利用大规模的前瞻性队列研究验证。Objective To explore the relationship between paternal age and neonatal gestational week age or rate of spontaneous preterm or early term.Methods A retrospective cohort study was conducted based on the pregnancy health care system and hospital information system of Tongzhou District Maternal and Child Health Hospital,in Beijing,and 13548 singleton newborns who were born vaginally and their parental information in prenatal health care during were enrolled.Results The mean birth age of the newborns was(39.0±1.4)weeks,among them,61.7%were full term(born at 39-40^(+6)gestational week age),4.0%were preterm(born at less than 37 gestational week age),and 26.1%were early term(born at 37-38^(+6)weeks gestational week age).The mean of paternal age at the conception was(30.0±4.5)years.Compared with neonates with paternal aged 20-25 years old,neonates with older fathers had higher risk of preterm and adjusted OR values from 1.74 to 2.45(95%confidence intervals were greater than 1),and the adjusted ORs for trend was 1.11(95%CI,1.06-1.15).There was no statistically significant association between early term birth rate and paternal age greater than 25 years.Conclusions Paternal age older than 25 years old might associated with increases the risk of preterm birth,large sample sized prospective cohort study are needed to validate the conclusion.
