摘 要:目的:采用液相色谱-质谱联用(LC-MS)技术,从代谢角度探讨摩腹对功能性消化不良(FD)大鼠的疗效及作用机制。方法:运用随机数字表法将30只大鼠分为3组:正常对照组、FD模型组和推拿组,每组10只。模型组和推拿组采用0.1%蔗糖碘乙酰胺溶液灌胃叠加小平台站立法建造FD模型。造模结束后运用苏木精-伊红(HE)染色观察胃窦组织病理变化。推拿组进行摩腹操作。干预7 d后,对各组大鼠取材,测定胃排空率,检测胃窦组织代谢物含量,通过相关分析筛选出差异代谢物,利用Metabo Analyst 5.0和KEGG在线数据库进行代谢通路分析。结果:HE染色结果显示,各组大鼠胃黏膜腺体排列整齐、结构完整,未见明显炎性细胞浸润,胃窦组织病理学检测无明显差异。与正常对照组相比,FD模型组大鼠胃排空率显著下降(P<0.05);与FD模型组相比,推拿组大鼠胃排空率显著升高(P<0.05)。代谢组学结果显示,与正常对照组相比,FD模型组胃窦组织中视黄醛、精氨酸琥珀酸、N-甲基-α-氨基异丁酸、乙酰磷酸酯、膦酰基乙酸酯、2-苯基丁酸、肌酸、4-羟基肉桂酸和苯丙酮酸含量显著升高,腺嘌呤、谷胱甘肽、腺苷、烟酰胺腺嘌呤二核苷酸、二磷酸腺苷核糖和水苏糖含量显著降低;与FD模型组相比,推拿组胃窦组织中单磷酸腺苷、精氨酸琥珀酸、L-谷氨酸、5-甲基胞嘧啶、甲硫腺苷、L-脯氨酸、鸟嘌呤、肌酸、焦谷氨酸、酪氨酸、谷氨酰胺、N-甲基烟酰胺和吲哚丙烯酸含量降低。结论:腹部推拿可改善FD大鼠胃排空率,调节胃组织代谢,其作用机制可能与精氨酸生物合成,丙氨酸、天冬氨酸和谷氨酸代谢,以及D-谷氨酰胺和D-谷氨酸代谢有关。AIM:To investigate the effects of abdominal Tuina on functional dyspepsia(FD)in rats,and to explore its mechanisms focusing on metabolism using liquid chromatography-mass spectrometry(LC-MS).METHODS:Thirty rats were randomly divided into 3 groups:normal control(NC)group,FD model group and Tuina group,with 10 rats in each group.The rats in FD model group and Tuina group were administered 0.1%sucrose iodoacetamide solution via gavage and subjected to the small platform standing method to induce the FD model.After modeling,gastric antrum tissue samples were obtained,and hematoxylin-eosin(HE)staining was used to observe histopathologic changes.Following 7 d of intervention,samples were collected from each group to measure gastric emptying rate,detect metabolite content in gastric antrum tissues,and screen for differential metabolites through correlation analysis.These differential metabolites were then subjected to further examination using MetaboAnalyst 5.0 and KEGG online database for metabolic pathway analysis.RESULTS:The HE staining results revealed that the gastric mucosa glands of the rats in all groups exhibited a well-organized arrangement with intact structures,no obvious inflammatory cell infiltration was observed,and no significant difference was observed in the histopathology of the gastric antrum.Compared with NC group,the gastric emptying rate of the rats in FD model group was significantly decreased(P<0.05).Conversely,the gastric emptying rate of the rats in Tuina group displayed a significant increase compared with FD model group(P<0.05).Metabolomics analysis revealed notable differences in the levels of various metabolites in gastric antrum tissues among different groups.In comparison with NC group,the rats in FD model group exhibited significant increases in the levels of retinal,argininosuccinic acid,N-methyl-α-aminoisobutyric acid,acetyl phosphate,phosphonoacetate,2-phenylbutyric acid,creatine,4-coumarate,and phenylpyruvate.Conversely,the levels of adenine,glutathione,adenosine,β-nicotinamide ad
