摘　　要：目的:探讨ESWAN及DTI序列在原发性高血压患者脑微出血(CMBs)中的定量评估价值。方法:收集43例患有CMBs的原发性高血压患者(EH)作为研究组,并将43例对侧正常大脑作为对照组,分别记录、测量位于基底节/丘脑、幕下、皮层-皮层下CMBs的数目及相应PV值、FA值、ADC值。对比分析各区域CMBs的PV值、FA值、ADC值之间的相关性。结果:在43例患有CMBs的高血压患者中发现病灶共计161个,以基底节/丘脑患病率最高,其次为幕下、皮层-皮层下;基底节/丘脑的PV值低于幕下及皮层-皮层下;各区域CMBs的PV值及FA值均低于对照组,而ADC值高于对照组;总体CMBs及基底节/丘脑区域CMBs的PV值与FA值呈正相关。结论:ESWAN及DTI序列可以对高血压患者CMBs的诊断和脑白质微观结构的损伤程度进行定量评估。Objective:To investigate the quantitative evaluation value of ESWAN and DTI sequences in cerebral microbleeds(CMBs)in patients with essential hypertension.Methods:Forty-three patients with essential hypertension(EH)with CMBs were enrolled as the study group,and forty-three contralateral normal brains as the control group.The number of CMBs located in basal ganglia/thalamus,subtentorial,and cortical to subcortical CMBs as well as their PV,FA and ADC values were recorded and measured respectively.The correlation between PV value,FA value and ADC value of CMBs in each region was analyzed.Results:A total of 161 lesions were found in 43 hypertensive patients with CMBs.The basal ganglia/thalamus had the highest prevalence,followed by subatentorial and subcortical lesions.The PV value of basal ganglia/thalamus was lower than that of subatentorial and subcortical.The PV and FA values of CMBs in each region were lower than those in the control group,while ADC values were higher than those in the control group.PV values of CMBs and FA values of CMBs in basal ganglia/thalamus region were positively correlated.Conclusion:ESWAN and DTI sequences can be used for diagnosis of CMBs and quantitative evaluation of damage degree of white matter microstructure in hypertensive patients.
