单侧输尿管结石导致肾绞痛的CT特征分析

摘　　要：目的:探讨单侧输尿管结石导致肾绞痛的CT特征。方法:选取北京市密云区中医医院2020年3月至2021年3月诊治的单侧输尿管结石导致肾绞痛的94例患者为观察组,另外,选取北京市密云区中医医院2020年3月至2021年3月40例泌尿系CT检查结石阴性患者为对照组。按输尿管生理性狭窄部位把观察组分为腹段组(32例)、盆段组(29例)、膀胱壁内段组(33例),并按视觉模拟疼痛量表(VAS)得分情况将观察组患者划分成轻度、中度、重度疼痛组,分别为26例、48例、20例,其中轻度的VAS评分为1-3分,中度的为4-6分,而重度的则为7-10分。观察其肾实质CT情况。结果:腹段、盆段、膀胱壁内段组的双肾CT比值分别为(83.62±3.28)%、(83.53±3.16)%、(83.72±3.38)%,较对照组的(95.46±3.89)%小,轻度疼痛组双肾CT比值为(87.64±4.53)%,较中度、重度疼痛组的(84.21±3.41)%、(81.13±2.89)%高,有统计学差异(P<0.05)。结论:对于单侧输尿管结石造成的肾绞痛患者,难以由双肾CT比值的降低对于结石的所在部位进行预估,但能够精准的对患者的疼痛情况进行反映。Objective:To investigate the CT characteristics of renal colic caused by unilateral ureteral stones.Methods:94 patients with unilateral ureteral stones from March 2020 to March 2021 were selected as the observation group,in addition,40 patients with urinary system CT stones negative from March 2020 to March 2021 in Miyun District Hospital of Traditional Chinese Medicine were selected as the control group.The observation groups were divided into abdominal(32),pelvic(29),inner segment of the bladder wall,(33),and into mild,moderate,and severe pain,26,48,20,with mild VAS,1-3,4-6 for moderate and 7-10 for severe cases.The renal parenchyma CT was observed.Results:Dual renal CT ratio of abdominal,pelvic,(83.62±and bladder wall 3.28)%,(83.53±3.16)%,(83.72±3.38)%,smaller than control(95.46±3.89)%,dual renal CT ratio(87.64±4.53)%than moderate and (84.21±2.89)%respectively(P<0.05).Conclusion:For patients with renal colic caused by unilateral ureteral stones,it is difficult to predict the stone location by the reduction of double renal CT ratio,but it can accurately reflect the pain situation of patients.
