摘  要:目的基于荧光原位杂交法的液态活检技术在肺癌早期诊断中的应用。方法使用3号与10号染色体的四色非放射性荧光着丝粒探针和端粒探针对120例肺结节血液样本的间期分裂细胞进行杂交染色检测,分析杂交后扫描所得的细胞图像,并计算其灵敏度、特异度和准确度。结果荧光原位杂交法检测120例血液样本中73例细胞内染色体分裂基本正常,循环肿瘤异常细胞(CTC)数<3,为阴性;47例细胞内3号或10号染色体出现双通道同时扩增,且CTC数≥3,为阳性。对应病理切片结果显示79例阴性,41例阳性。荧光原位杂交法灵敏度、特异度与准确度分别为80.49%、89.04%、81.67%。结论荧光原位杂交法灵敏度与准确度较高,液态活检技术可为临床肺癌早期诊断提供新的依据。Objective To explore the application of liquid biopsy based on fluorescence in situ hybridization method in early diagnosis of lung cancer.Methods Interphase cells of blood sample from pulmonary nodules of 120 cases were hybridized and stained using a four-color nonradioactive fluorescent chromosome enumeration probe and telomere probe on chromosomes 3 and 10.The cell images obtained by scanning after hybridization were analyzed and their sensitivity,specificity and accuracy were calculated.Results Among 120 cases of blood samples subject to fluorescence in situ hybridization,73 cases had normal intracellular chromosome division,with the number of CTC<3,which was negative;47 cases had dualchannel amplification on chromosome 3 or 10,with CTC number≥3,which was positive.Corresponding pathological section results showed negative in 79 cases and positive in 41 cases.The sensitivity,specificity and accuracy of fluorescence in situ hybridization were 80.49%,89.04% and 81.67%,respectively.Conclusion Fluorescence in situ hybridization has high sensitivity and accuracy.Liquid biopsy can provide a new basis for the early diagnosis of lung cancer.
