摘　　要：目的探讨破裂颅内动脉瘤患者介入术后脑血管痉挛(CVS)的影响因素。方法回顾性分析2019年6月至2020年10月本院68例行介入术治疗的破裂颅内动脉瘤患者的临床资料,统计患者术后CVS发生情况,并收集临床资料分析破裂颅内动脉瘤患者介入术后CVS发生的影响因素。结果68例破裂颅内动脉瘤患者CVS发生16例,发生率为23.53%;CVS发生患者中合并高血压占比,术前Fisher分级为Ⅲ~Ⅳ级占比均高于未发生患者,既往颅内动脉瘤破裂次数多于未发生患者,差异有统计学意义(P<0.05)。Logistic回归分析发现,高血压、术前Fisher分级为Ⅲ~Ⅳ级、既往颅内动脉瘤破裂次数多是破裂颅内动脉瘤患者介入术后发生CVS的影响因素(OR>1,P<0.05)。结论破裂颅内动脉瘤介入术后有一定的CVS发生率,而高血压、术前Fisher分级为Ⅲ~Ⅳ级、既往颅内动脉瘤破裂次数高均是发生CVS的影响因素。Objective To investigate the influencing factors of cerebral vasospasm(CVS)in patients with ruptured intracranial aneurysm after interventional surgery.Methods The clinical data of 68 patients with ruptured intracranial aneurysm who underwent interventional treatment in our hospital from June 2019 to October 2020 were retrospectively analyzed.The incidence of postoperative CVS in the patients was counted.Cilinical data were collected to analyze the ruptured intracranial artery.Factors influencing the occurrence of CVS after interventional surgery in patients with tumors.Results There were 16 cases of CVS in 68 patients with ruptured intracranial aneurysm,with an incidence rate of 23.53%;the incidence of hypertension in the incidence patients was higher than that of the non-occurring patients;the number of aneurysm ruptures was higher than that of the non-occurring patients,and the difference was statistically significant(P<0.05);Logistic regression analysis found that hypertension,preoperative Fisher's classification wasⅢ-Ⅳ,and the high number of ruptures of previous intracranial aneurysms might be the influencing factors of CVS in patients with ruptured intracranial aneurysm after interventional therapy(OR>1,P<0.05).Conclusion There is a certain incidence of CVS in ruptured intracranial aneurysms after interventional procedures,hypertension,preoperative Fisher classification ofⅢ-Ⅳ,and high number of ruptured intracranial aneurysms are all influential factors for the occurrence of CVS.
