摘　　要：目的评价脉络宁联合西医治疗慢性肺源性心脏病急性期的有效性。方法检索PubMed、WebofScience、Cochrance、中国知网、中国生物医学数据库、万方数据库、维普数据库中脉络宁治疗肺心病临床研究的相关文献,检索时间为2000年至2021年8月15日,由2名研究者筛选文献并进行质量评价,纳入合格文献,采用RevMan5.3进行Meta分析。结果最终共纳入分析文献9篇,脉络宁联合西医可有效治疗慢性肺源性心脏病急性期,实验组治疗有效率高于对照组,差异有统计学意义[OR=4.82,95%CI(3.14,7.40),P<0.00001];实验组红细胞压积低于对照组,差异有统计学意义[MD=-8.09,95%CI(-12.32,-3.86),P=0.0002];实验组纤维蛋白原水平低于对照组,差异有统计学意义[MD=-0.75,95%CI(-0.90,-0.59),P<0.00001];实验组血浆黏度水平低于对照组,差异有统计学意义[MD=-0.13,95%CI(-0.20,-0.06),P=0.001];两组红细胞压积比较差异无统计学意义[MD=-4.9,95%CI(-9.98,0.19),P=0.06]。结论脉络宁联合西医治疗肺心病急性发作期临床疗效显著,但仍需更多研究证实和补充。Objective To evaluate the effectiveness of Mailuoning combined with Western medicine in the treatment of acute exacerbation with chronic cor pulmonale.Methods PubMed,Web of Science,Cochrance,China National Knowledge Infrastructure,China Biology Medicine disc,WanFang date,China Science and Technology Journal Database were searched for relevant literatures on the clinical research of Mailuoning in the treatment of pulmonary heart disease,the retrieval period was from 2000 to August 15,2021,the literature was screened and evaluated by two researchers,qualified literatures were included and Meta-analysis was performed using Rev Man 5.3.Results A total of 9 literatures were included in the analysis,Mailuoning combined with Western medicine can effectively treat the acute stage of chronic pulmonary heart disease,the effective rate in the experimental group was higher than that in the control group,the difference was statistically significant(OR=4.82,95%CI[3.14,7.40],P<0.00001);the level of hematocrit in the experimental group was lower than that of the control group,the difference was statistically significant(MD=-8.09,95%CI[-12.32,-3.86],P=0.0002);the plasma fibrinogen in the experimental group was lower than that in the control group,the difference was statistically significant(MD=-0.75,95%CI[-0.90,-0.59],P<0.00001);and the level of plasma viscosity in the experimental group was lower than that in the control group,the difference was statistically significant(MD=-0.13,95%CI[-0.20,-0.06],P=0.001);there was no significant difference in hematocrit between the two groups(MD=-4.9,95%CI[-9.98,0.19],P=0.06).Conclusion Mailuoning combined with Western medicine in the treatment of acute attack of pulmonary heart disease has significant clinical effect,but it still needs more research to confirm and supplement.
