微创跟腱吻合器手术与无皮肤切口修复术治疗跟腱断裂的临床研究

摘　　要：目的探讨微创跟腱吻合器手术与无皮肤切口修复术治疗跟腱断裂的疗效。方法选取2019年5月至2021年2月本院收治的80例跟腱断裂患者作为研究对象,按手术方式不同分为对照组与观察组,每组40例。对照组实施无皮肤切口修复术治疗,观察组实施微创跟腱吻合术治疗,比较两组临床疗效、视觉模拟评分法(VAS)评分、踝关节后足功能(AOFAS)评分、并发症发生率。结果两组治疗总优良率比较差异无统计学意义。治疗前,两组VAS评分、AOFAS评分比较差异无统计学意义;治疗后,两组VAS评分低于治疗前,AOFAS评分高于治疗前,且观察组VAS评分低于对照组,AOFAS评分高于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较差异无统计学意义。结论微创跟腱吻合器手术与无皮肤切口修复术治疗的效果相当,而微创跟腱吻合器手术关节功能改善程度更佳,且患者的疼痛得到较好的改善,在实际的临床诊疗中,需根据患者实际情况选择更有利的治疗方案。Objective To investigate the efficacy of minimally invasive Achilles tendon stapler and skin-free repair in the treatment of Achilles tendon rupture.Methods A total of 80 patients with Achilles tendon rupture admitted to our hospital from May 2019 to February 2021 were selected as the research subjects,and they were divided into the control group and the observation group by different surgical method,with 40 cases in each group.The control group was treated with skin-free repair,and the observation group was treated with minimally invasive achilles tendon stapler,and the clinical efficacy,visual analogue scale(VAS)score,american orthopedic foot and ankle society(AOFAS)score,and complication rate were compared between two groups.Results There was no significant difference in the total good rate between the two groups.Before treatment,there was no significant difference in VAS score and AOFAS score between the two groups;after treatment,the VAS score of two group were lower than that before treatment,and AOFAS score was higher than that before treatment,and the VAS score in the observation group was lower than that in the control group,and AOFAS score was higher than control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups.Conclusion The clinical effect of minimally invasive Achilles tendon stapler is similar to that of repair without skin incision,but the minimally invasive Achilles tendon stapler has better joint function improvement and better pain improvement effect,in the actual clinical diagnosis and treatment,more favorable treatment plan should be selected according to the actual situation of patients.
