双侧同期全膝关节置换术和单侧分期全膝关节置换术治疗膝骨性关节炎的临床疗效

摘　　要：目的探究双侧同期全膝关节置换术和单侧分期全膝关节置换术治疗膝骨性关节炎的临床疗效。方法选取2019年3月至2021年5月本院收治的80例膝骨性关节炎患者作为研究对象,按照手术介入方式不同分为单侧组与双侧组,每组40例。单侧组行单侧分期全膝关节置换术,双侧组行双侧同期全膝关节置换术,比较两组并发症发生情况、肢体功能、日常生活能力、社交能力、生命质量。结果两组并发症发生率比较差异无统计学意义。干预后,两组上肢功能、下肢功能评分均高于干预前,双侧组下肢功能评分均高于单侧组,差异有统计学意义(P<0.05),两组上肢功能评分比较差异无统计学意义。干预后,两组改良Barthel指数(MBI)评分、社会支持评定量表(SSRS)评分均高于干预前,且双侧组高于单侧组,差异有统计学意义(P<0.05)。双侧组生理领域、心理领域、精神领域、社会领域、环境领域、独立领域、总体健康评分均高于单侧组,差异有统计学意义(P<0.05)。结论膝骨性关节炎行双侧同期全膝关节置换术的临床应用价值高于单侧分期全膝关节置换术,有利于促进患者生理状态恢复,提高预后生命质量,值得临床推广应用。Objective To explore the clinical efficacy of bilateral simultaneous total knee arthroplasty and unilateral staged total knee arthroplasty in the treatment of knee osteoarthritis.Methods 80 cases of knee osteoarthritis treated in our hospital from March 2019 to May 2021 were selected as the research subjects,and they were divided into unilateral group and bilateral group according to different surgical intervention methods,with 40 cases in each group.Unilateral group underwent unilateral staged total knee arthroplasty,and bilateral group underwent bilateral simultaneous total knee arthroplasty,the complications,limb function,daily living ability,social skills and quality of life were compared between the two groups.Results There was no significant difference in the incidence of complications between the two groups.After intervention,the scores of upper limb function and lower limb function in the two groups were higher than those before intervention,and the scores of lower limb function in the bilateral group were higher than those in the unilateral group,the differences were statistically significant(P<0.05),but there was no statistical difference of uplimb function scores between the two groups.After intervention,the scores of modified Barthel index(MBI)and social support rating scale(SSRS)of the two groups were higher than those before intervention,and the bilateral group were higher than those of unilateral group,the differences were statistically significant(P<0.05).The scores of bilateral physiological,psychological,spiritual,social,environmental,independent and overall health in the bilateral group were higher than those in the unilateral group,the differences were statistically significant(P<0.05).Conclusion The clinical application value of bilateral simultaneous total knee arthroplasty for knee osteoarthritis is higher than that of unilateral staged total knee arthroplasty,which is conducive to promoting the recovery of patients'physiological state and improving the quality of life of prognosis,and is w
