腹腔镜下肝左外叶切除术治疗肝内胆管结石的临床疗效

摘　　要：目的探究腹腔镜下肝左外叶切除术治疗肝内胆管结石的临床疗效。方法选取2019年10月至2020年9月本院收治的74例肝内胆管结石患者作为研究对象,按其自行选取的手术方式分为参照组与研究组,每组37例。参照组行开腹肝左外叶切除术,研究组行腹腔镜下肝左外叶切除术,比较两组手术时间、切口长度、术中出血量、通气时间、卧床时间、术后住院时间、并发症发生情况。结果研究组手术时间、切口长度均短于参照组,术中出血量少于参照组,差异有统计学意义(P<0.05)。研究组通气时间早于参照组,卧床时间、术后住院时间均短于参照组,差异有统计学意义(P<0.05)。研究组并发症发生率为5.40%,低于参照组的21.61%,差异有统计学意义(P<0.05)。结论腹腔镜下肝左外叶切除术治疗肝内胆管结石效果显著,有利于减少患者创伤,降低并发症发生率,促进其康复,值得临床推广应用。Objective To analyze the clinical efficacy of laparoscopic left lateral hepatectomy in the treatment of intrahepatic bile duct stones.Methods A total of 74 patients with intrahepatic bile duct stones admitted to our hospital from October 2019 to September 2020 were selected as the research subjects,and they were divided into reference group and study group according to their own selected surgical methods,with 37 cases in each group.The reference group underwent open left lateral hepatectomy,and the study group underwent laparoscopic left lateral hepatectomy,the operation time,incision length,intraoperative blood loss,ventilation time,bed rest time,postoperative hospital stay and complications were compared between the two groups.Results The operation time and incision length of the study group were shorter than those of the reference group,and the intraoperative blood loss was less than that of the reference group,the differences were statistically significant(P<0.05).The ventilation time of the study group was earlier than that of the reference group,and the bed resttime and postoperative hospital stay were shorter than those of the reference group,the differences were statistically significant(P<0.05).The incidence of complications in the study group was 5.40%,which was lower than 21.61%in the reference group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic left lateral hepatectomy is effective in the treatment of intrahepatic bile duct stones,which is beneficial to reduce the trauma of patients,reduce the incidence of complications,and promote their rehabilitation,it is worthy of clinical application.
