摘 要:糖尿病肾病是糖尿病所致的肾小球、肾小管损伤而引起的一种进行性肾脏疾病,是2型糖尿病最主要的慢性微血管并发症之一,目前已成为全球导致各种终末期肾脏疾病最主要的原因之一,由于其发病机制十分复杂,临床尚无有效治疗措施干预或延缓糖尿病肾病的发生、发展。随着近年来对维生素D及其受体的深入研究发现,其除在调节钙磷代谢的经典作用外,许多临床及动物研究中发现维生素D缺乏与糖尿病肾病存在一定的相关性,维生素D可通过抑制肾素-血管紧张素系统、抗纤维、保护足细胞、抗氧化应激、抗炎、调节免疫、改善胰岛素抵抗等多个途径延缓糖尿病肾病的进展,本文旨在对维生素D在糖尿病肾病中的作用及可能机制进行阐述。Diabetic nephropathy is progressive kidney disease caused by the damage of glomeruli and renal tubules resulted from diabetes,which is one of the most important chronic microvascular complications of type 2 diabetes.It has become the most common cause of various end-stage renal diseases in the world.Due to its complex pathogenesis,there is currently no effective treatment to interfere or suppress the occurrence and development of diabetic nephropathy.With the in-depth study of vitamin D and its receptors in recent years,it has been found that in addition to its classical role in regulating calcium and phosphorus metabolism,many clinical and animal experiments have observed a certain correlation between vitamin D deficiency and diabetic nephropathy,vitamin D is able to delay the progression of diabetic nephropathy in many ways such as inhibiting the renin-angiotensin system,anti-fibrosis,protecting podocytes,resisting oxidative stress,anti-inflammatory,regulating immunity,improving insulin resistance,this article describes the role and possible mechanism of vitamin D in diabetic nephropathy.
