摘　　要：目的探讨数字减影血管造影(DSA)引导下半月神经节球囊压迫术治疗三叉神经痛的近远期疗效及安全性。方法选取2019年1月至2022年2月九江学院附属医院收治的60例顽固性三叉神经痛患者作为研究对象。患者均接受DSA引导下半月神经节球囊压迫术治疗,治疗后随访3~12个月,比较患者治疗前后视觉模拟评分法(VAS)、手术治疗效果与并发症发生情况。结果患者术后第3个月(T_(2))和术后第12个月(T_(3))VAS评分均低于治疗前(T_(0))和出院时(T_(1)),差异有统计学意义(P<0.05);60例患者中,临床治愈18例,显效20例,有效20例,无效2例,治疗总有效率为96.67%;出现患侧面部感觉迟钝45例,同侧咀嚼乏力8例,口唇部疱疹18例,咀嚼肌功能障碍1例,未发生严重并发症。结论DSA引导下半月神经节球囊压迫术治疗三叉神经痛近远期疗效均较为理想,且安全可靠,值得临床推广应用。Objective To investigate the shortterm and longterm efficacy and safety of digital subtraction angiography(DSA)guided semilunar ganglion balloon compression in the treatment of trigeminal neuralgia.Methods 60 patients with intractable trigeminal neuralgia admitted to the Affiliated Hospital of Jiujiang University from January 2019 to February 2022 were selected as the research subjects.All patients received DSAguided semilunar ganglion balloon compression,and they were followed up for 3 to 12 months,the visual analogue scale(VAS),efficacy and complications were compared before and after treatment.Results The VAS scores of patients at 3 months(T_(2))and 12 months(T_(3))after operation were lower than those before treatment(T_(0))and at discharge(T_(1)),and the difference was statistically significant(P<0.05).Among the 60 patients,18 cases were clinically cured,20 cases were markedly effective,20 cases were effective,and 2 cases were ineffective,the total effective rate was 96.67%.There were 45 cases of sensory retardation on the affected side,8 cases of ipsilateral chewing fatigue,18 cases of herpes on the lips,and 1 case of masticatory muscle dysfunction,no serious complications occurred.Conclusion DSA-guided semilunar ganglion balloon compression in the treatment of trigeminal neuralgia short-term and long-term efficacy are ideal,safe and reliable,which is worthy of clinical promotion and pplication.
