摘　　要：目的分析超声造影(CEUS)联合声触诊组织成像(VTI)技术鉴别甲状腺结节(TNS)性质的临床价值。方法选取2019年1—12月江西中医药大学附属医院收治的61例TNS患者作为研究对象,均接受CEUS及VTI检查,以病理结果为金标准,分析CEUS联合VTI鉴别TNS性质的一致性及灵敏度、特异度及准确度。结果病理诊断结果:61例TNS患者中,共检出66个结节,其中恶性结节28个(42.42%),良性结节38个(57.58%)。CEUS与病理金标准诊断TNS性质的Kappa值为0.66,一致性一般;VTI与病理金标准诊断TNS性质的Kappa值为0.44,一致性一般;CEUS联合VTI与病理金标准诊断TNS性质的Kappa值为0.82,一致性好。CEUS联合VTI诊断TNS性质的灵敏度、准确度均高于CEUS、VTI单独诊断,差异有统计学意义(P<0.05)。结论CEUS与VTI均可对TNS的性质进行鉴别诊断,但二者联合诊断的诊断效能更高,值得临床推广应用。Objective To analyze the clinical value of contrast-enhanced ultrasound(CEUS)combined with virtual touch tissue imaging(VTI)in distinguishing the nature of thyroid nodules(TNS).Methods 61 patients with TNS admitted to the Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine from January to December 2019 were selected as the study subjects,all of them underwent CEUS and VTI examina-tions.The consistency,sensitivity,specificity and accuracy of CEUS combined with VTI in identifying the nature of TNS were analyzed based on the pathological results as the gold standard.Results Pathological diagnosis results:among the 61 patients with TNS,66 nodules were detected,there were 28 malignant nodules(42.42%)and 38 benign nodules(57.58%).The Kappa value of CEUS and pathological gold standard in diagnosing the nature of TNS was 0.66,and the consistency was general;the Kappa value of VTI and pathological gold standard in diagnosing the nature of TNS was 0.44,and the consistency was general;the Kappa value of CEUS combined with VTI and pathological gold standard in diagnosing the nature of TNS was 0.82,and the consistency was good.The sensitivity and accuracy of CEUS combined with VTI in diagnosing the nature of TNS were high-er than those of CEUS and VTI alone,and the differences were statistically significant(P<0.05).Conclusion Both CEUS and VTI can differentiate the nature of TNS,but the combined diagnosis of CEUS and VTI is more effective,and is worthy of clinical promotion and application.
