健脾益肾泄浊方辅助治疗对持续性非卧床腹膜透析脾肾两虚湿浊证患者残余肾功能的影响——随机对照试验

摘　　要：目的观察健脾益肾泄浊方辅助治疗对持续性非卧床腹膜透析(CAPD)脾肾两虚湿浊证患者残余肾功能(RRF)的影响。方法将70例CAPD脾肾两虚湿浊证患者随机分为治疗组和对照组各35例。对照组予CAPD加基础治疗,治疗组患者在对照组治疗基础上加用健脾益肾泄浊方口服治疗,每日1剂,两组疗程均为24周。比较两组治疗前及治疗12周、24周RRF,治疗12周、24周时尿量下降速率,并观察安全性。结果治疗12周和24周,治疗组RRF分别为2.55(1.71,3.90)、2.40(1.41,3.41)ml/min,对照组分别为1.65(0.72,2.65)、1.33(0.63,2.69)ml/min,治疗组均明显高于对照组(P<0.05)。与本组治疗前比较,对照组治疗12周和24周RRF均明显下降,而治疗组仅治疗24周有明显下降(P<0.05)。治疗24周治疗组尿量下降速率低于对照组(P<0.05)。治疗过程中对照组发生1例高钾血症,治疗组无不良反应发生。结论健脾益肾泄浊方辅助治疗可延缓CAPD脾肾两虚湿浊证患者的RRF下降,对RRF具有保护作用。Objective To observe the effect of Jianpi Yishen Xiezhuo Formula(健脾益肾泄浊方,JYXF)on residual renal function(RRF)in patients with continuous ambulatory peritoneal dialysis(CAPD)of spleen-kidney deficiency and damp-turbidity syndrome.Methods Seventy CAPD patients with spleen-kidney deficiency and damp-turbidity syndrome were randomly divided into treatment group and control group,35 cases each.The control group was treated with CAPD plus basic treatment,while the treatment group was given JYXF once a day additionally.The course of treatment in both groups was 24 weeks.RRF before and at 12-and 24-week treatment,urine volume decline rate at 12-and 24-week treatment were compared between groups,and the safety was observed.Results The RRF of the treatment group was significantly higher than that of the control group at 12 weeks(2.55 ml/min,[1.71,3.9]vs 1.65 ml/min,[0.72,2.65])and at 24 weeks(2.40 ml/min,[1.41,3.41]vs 1.33 ml/min,[0.63,2.69],P<0.05).After treatment,RRF in the control group decreased significantly at 12 and 24 weeks,while RRF in the treatment group decreased significantly at 24 weeks(P<0.05).The decline rate of urine volume in the treatment group was lower than that in the control group at 24 weeks(P<0.05).During the treatment,one case of hyperkalemia occurred in the control group,and there was no adverse events in the treatment group.Conclusion The adjuvant therapy of JYXF can delay the RRF decline in CAPD patients of spleen-kidney deficiency and damp-turbidity syndrome,and has a protective effect on RRF.
