摘 要:目的:采用7T心脏磁共振组织追踪成像(CMR-TT)评估苍艾挥发油对盐酸异丙肾上腺素(ISO)诱导的慢性心力衰竭(CHF)大鼠心脏结构和功能的保护作用。方法:32只SD雄性大鼠连续14d皮下多点注射ISO10mg·kg^(-1)·d^(-1)后,普通饲养4周,构建CHF大鼠模型。造模结束后将存活的24只模型大鼠随机分为空白实验组、苍艾挥发油组、麝香保心丸组,再另设正常对照组,每组8只。各组连续给予相应药物或等量0.9%氯化钠溶液灌胃28d,灌胃结束后行7TCMR电影序列扫描。应用后处理软件CVI42分析得到左心室结构和功能参数、左心室心肌应变参数。磁共振扫描结束后处死大鼠,摘取心脏行病理学检查。以Pearson相关分析评价左心室整体心肌应变与心肌损伤评分、心肌组织胶原容积分数(CVF)间的相关性。结果:与正常对照组比较,空白实验组大鼠左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)、心肌损伤评分、CVF显著升高(P<0.01),而左心室每搏输出量(LVSV)、左心室射血分数(LVEF)、左心室整体径向应变(LVGRS)、左心室整体环向应变(LVGCS)、左心室整体纵向应变(LVGLS)均显著降低(P<0.05,P<0.01),病理染色显示大面积弥漫性的心肌组织纤维化,并有大量的炎症细胞浸润;苍艾挥发油组则明显提高CHF大鼠的LVEF、LVGRS、LVGCS、LVGLS(P<0.01),同时心肌损伤和心肌纤维化程度也得到明显改善。相关性研究中,LVGLS与CVF间的相关性最高(r=0.6844,P<0.01)。结论:苍艾挥发油可明显改善CHF大鼠心肌损伤和心肌纤维化程度,对大鼠心脏结构和功能具有一定的保护作用。Objective:The protective effect of Cang-ai volatile oil on cardiac structure and function in rats with chronic heart failure(CHF)induced by isoproterenol hydrochloride(ISO)was evaluated by 7T cardiovascular magnetic resonance tissue tracking imaging(CMR-TT).Methods:Thirty-two SD male rats were randomly reared for 4 weeks after subcutaneous multipoint injection of ISO 10 mg·kg^(-1)·d^(-1)saline for 14 consecutive days to construct a CHF rat model.After the establishment of the model,the surviving 24 rats were randomly divided into blank experimental group,Cang-ai volatile oil group and Shexiang Baoxin Pill group,and the additional normal control group(8 rats in each group).Each group was given the corresponding medicine or the same amount of normal saline intragastrical for 28 days.After intragastric administration,7T CMR film sequence was scanned.The left ventricular structure and function parameters and left ventricular myocardial strain parameters were analyzed by post-processing software CVI42.After magnetic resonance scanning,the rats were killed,and the hearts were removed for pathological examination.Pearson correlation analysis was used to evaluate the correlation between left ventricular global myocardial strain and myocardial injury score and collagen volume fraction(CVF).Results:Compared with the normal control group,left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),myocardial injury score and CVF in the blank experimental group were significantly increased(P<0.01),while left ventricular stroke volume(LVSV),left ventricularejection fraction(LVEF),left ventricular global radial strain(LVGRS),left ventricular global circumferential strain(LVGCS)and left ventricular global longitudinal strain(LVGLS)were decreased(P<0.05,P<0.01).Pathological staining showed large area diffuse myocardial fibrosis and a large amount of inflammatory cell infiltration.In the Cang-ai volatile oil group,LVEF,LVGRS,LVGCS and LVGLS of CHF rats were significantly increased(P<0.01),and the deg
