基于营卫理论探讨昼夜节律紊乱与糖尿病视网膜病变的相关性

摘　　要：糖尿病视网膜病变(DR)是糖尿病最为常见的微血管并发症,其发病机制尚未完全阐明。近年研究发现,昼夜节律紊乱与DR的发生发展密切相关,表明针对昼夜节律的调控可能是一种减缓DR进展的潜在治疗策略。“卫主气,营主血”,营卫理论长期指导临床实践,是中医辨病辨证、依证选方、遣方用药的重要依据。文章从中医营卫理论的角度对DR发展过程中昼夜节律紊乱的作用进行探讨,归纳DR昼夜节律紊乱从营卫论治的治疗原则,旨在为临床防治DR提供新的视角和理论依据。Diabetic retinopathy(DR)is the most common microvascular complication of diabetes,and its pathogenesis has not been fully clarified.Recent studies have proved that circadian rhythm disorder is closely related to the occurrence and development of DR,indicating that the regulation of circadian rhythm function may be a potential therapeutic strategy to slow down the progress of DR.Wei governs qi and ying governs blood'.Theory of nutrient qi and defensive qi guides clinical practice for a long time.It is an important basis for disease differentiation and syndrome differentiation,the selection of formula according to syndrome differentiation,and the medication according to formula.This paper discusses the role of circadian rhythm disorder in the development of DR from the perspective of the theory of nutrient qi and defensive qi,and summarizes the treatment principles of circadian rhythm disorder of DR from ying and wei,aiming at providing a new perspective and theoretical basis for clinical prevention and treatment of DR.
