论灸法的“壮”“疮”与“效”

摘　　要：灸法的核心是灸效,根据疾病情况不同选择足够的刺激量,决定灸法、灸时、穴位、艾炷大小等。古人以灸壮代灸量,“壮”本身含有“烧灼致创”之意,又有“灸疮必发”之说,说明刺激量导致的穴位“疮伤”是灸法取效的关键。通过对直接灸(化脓灸、非化脓灸)、隔物灸、实按灸、温和灸的使用特点、灸量范围及作用机制进行研究,具体阐述灸法的“壮”“疮”与“效”的关系,发现灸法演变的总体趋势是在增强疗效的同时,减少壮数以及“疮伤”。在研究灸法问题时,我们应当紧扣关键问题,立足于中医学说,顺应时代潮流,强调灸须足量,减“疮伤”增“疗效”,厘清灸法的量效关系,在传承灸法精华的基础上,发展创新,以提高灸法疗效。The core of moxibustion is moxibustion effect,which is determined by selecting sufficient stimulation amount according to different disease conditions,including moxibustion method,moxibustion time,acupoints,and moxa cone size.The ancients used moxibustion to replace the amount of moxibustion,and the term Zhuang'itself implies the meaning of'burning to cause injury',as well as the saying that'moxibustion sores must occur',indicating that the stimulation caused by the amount of stimulation is the key to the effectiveness of moxibustion.By studying the usage characteristics,range of moxibustion dosage,and mechanism of action of direct moxibustion(purulent moxibustion,non purulent moxibustion),separated moxibustion,practical moxibustion,and mild moxibustion,the relationship between Zhuang',‘sore',and'effect'of moxibustion is specifically elaborated.It is found that the overall trend of moxibustion evolution is to enhance therapeutic effect while reducing the number of strengthening and'sore injury'.In the study of moxibustion,we should focus on key issues,base ourselves on the theory of traditional Chinese medicine,conform to the trend of the times,emphasize the sufficiency of moxibustion,reduce'sore'and increase'curative effect',clarify the relationship between the quantity and effect of moxibustion,and develop and innovate on the basis of inheriting the essence of moxibustion to improve the curative effect of moxibustion.
