加味桃核承气汤对单侧输尿管梗阻大鼠肾功能的保护作用

摘　　要：目的:探讨加味桃核承气汤对单侧输尿管梗阻大鼠肾功能的保护作用。方法:将40只SD雄性大鼠随机分为假手术组、模型组、依那普利组、中药组,每组10只。除假手术组外,其他组别大鼠均采用单侧输尿管结扎法建立单侧输尿管梗阻模型。假手术组除不结扎输尿管外,手术步骤与其他组别相同。术后第1天开始,中药组给予加味桃核承气汤12.18g·kg^(-1)·g^(-1)灌胃,依那普利组给予依那普利10mg·kg^(-1)·g^(-1)灌胃,模型组和假手术组给予等体积0.9%氯化钠溶液灌胃,1次/d,共14d。观察大鼠生存质量,给药14d后处死大鼠,检测大鼠肾脏脏器系数、24h尿蛋白定量、血清肌酐、血清尿素氮,通过HE、Masson染色观察梗阻侧肾脏病理变化。结果:与假手术组比较,模型组大鼠肾脏脏器系数、24h尿蛋白、血肌酐和尿素氮显著升高(P<0.05);与模型组比较,依那普利组、中药组大鼠肾脏脏器系数、24h尿蛋白、血肌酐和尿素氮显著降低(P<0.05);模型组大鼠肾小管上皮细胞变性,肾间质炎症细胞浸润,纤维组织增生,各治疗组大鼠肾脏病理改变均明显改善。结论:加味桃核承气汤能改善单侧输尿管梗阻大鼠生存质量,减少尿蛋白丢失,减少血清肌酐、尿素氮潴留,减轻肾脏病理损伤,保护肾功能。Objective:To observe the renal function protective effect of Modified Taohe Chengqi Decoction on rats with unilateral ureteral ligation.Methods:Forty male SD rats were randomly divided into sham operation group,model group,enalapril group and traditional Chinese medicine(TCM)group,with 10 rats in each group.Except the sham operation group,unilateral ureteral ligation model was established by unilateral ureteral obstruction(UUO)in other groups.The procedure of sham operation group was the same as other groups except that the ureter was not ligatured.On the lst day after postoperative,the rats in TCM group were given 12.18 g·kg^(-1)·d^(-1)of Modified Taohe Chengqi Decoction by intragastrical administration,the rats in enalapril group were given 10 m·kg^(-1)·d^(-1)of enalapril,the rats in the model group and the sham operation group were given equal volume gavage of normal saline,once a day,for a total of 14 days.The life quality of the rats was observed,and the rats were sacrificed 14 days later,The kidney index,24 h urine protein,serum creatinine and serum urea nitrogen of the rats were detected.The renal tissue of the obstructive side was collected for HE and Masson staining,and the pathological changes of renal tissue were observed.Results:Compared with the sham operation group,kidney index,24 h urine protein,serum creatinine and urea nitrogen were significantly increased in the model group(P<0.05).Compared with the model group,kidney index 24 h urine protein,serum creatinine and urea nitrogen were decreased significantly,in the enalapril group and the TCM group(P<0.05).In the model group,renal tubular epithelial cells were denaturated,interstitial inflammatory cells infiltrated,and fibrous tissue hyperplasia appeared.The renal pathological changes of rats in all treatment groups were significantly improved.Conclusion:Modified Taohe Chengqi Decoction can improve the quality of life of UUO rats,reduce the loss of urinary protein,the retention of serum creatinine and urea nitrogen,alleviate the renal pathologi
