功能性子宫出血患者力敏腧穴压痛阈的定量研究

摘　　要：目的:观察力敏腧穴在功能性子宫出血(DUB)患者体表分布及其压痛阈,探讨DUB患者力敏腧穴分布情况及客观表征。方法:DUB组和健康组每组各70例,参照力敏腧穴探查方法配合手持测痛仪测量压痛阈值,记录力敏腧穴频次,选取健康组相应腧穴测量压痛阈值,比较两组力敏腧穴发生率及腧穴压痛阈,分析DUB组力敏腧穴在不同经脉与部位上的差异。结果:健康组力敏腧穴发生率为12.86%,DUB组为95.71%,DUB组力敏腧穴归经主要为足太阴脾经、足太阳膀胱经及任脉,所属神经节段主要在T2-T4,T8-T12,L2-L5及S2,其中属于生殖系统相关神经节段(T10-S2)的力敏化腧穴占比为74.93%;力敏腧穴区按照平均压痛阈值从高到低排列依次为:风市、委中、承山、天枢、足三里、三阴交、环跳、中脘、阴陵泉、膻中、阴廉、血海,与健康组双侧对应穴区相比,DUB组腧穴压痛阈值的平均阈值显著偏低(P<0.01);DUB患者各所属经脉平均压痛阈值由高到低排序:督脉>足三阳经>任脉>足三阴经。DUB患者位于足三阴经上的力敏腧穴压痛阈值显著低于足三阳经与督脉(P<0.05),位于任脉上的力敏腧穴压痛阈值低于足三阳经及督脉(P<0.05);按照腧穴区平均压痛阈值在不同部位由高到低排列:腰背部>大腿部>小腿部>胸腹部,小腿部力敏腧穴压痛阈值显著低于腰背部和大腿部(P<0.05),胸腹部力敏腧穴压痛阈值显著低于小腿部、大腿部和腰背部(P<0.05)。结论:腧穴敏化现象与机体状态高度相关,DUB患者力敏腧穴发生率更高,其力敏腧穴分布与生殖系统相关神经节段(T10~S2)密切相关;DUB患者对机械力引起的疼痛较健康人更敏感,在不同经脉和部位的压痛阈存在差异,主要表现为左右失衡。Objective:To observe the distribution of forced-sensitive acupoints on the surface of patients with dysfunctional uterine bleeding(DUB)and their tenderness threshold,and to explore the distribution and objective characterization of acupoints on the surface of patients with DUB.Methods:70 DUB patients and 70 healthy subjects were included in each group,reference to forced-sensitive acupoints detection method with a handheld pain gauge measuring tenderness threshold,record the forced-sensitive acupoints occurrence frequency,corresponding acupoints in measuring pressure threshold selecting health groups,compared two groups of forced-sensitive acupoints rate and their tenderness threshold of acupoints,analysis of forced-sensitive DUB group differences in different meridians and traditional acupoints.Results:The sensitization rate of acupoints in the healthy group was 12.86%,and that in the DUB group was 95.71%.The meridians of acupoints in the DUB group were mainly the spleen meridians of foot Taiyin,the bladder meridians of foot Taiyang and the Ren Meridian,and the nerve segments were mainly T2-T4,T8-T12,L2-L5 and S2.Among them,the proportion of forced-sensitized acupoints belonging to reproductive system related segments(T10-S2)was 74.93%.The average tenderness threshold in the area of forced-sensitive acupoints was ranked from high to low:The average threshold of acupoint pain in Fengshi(GB 31),Weizhong(BL 40),Chengshan(BL 57),Tianshu(ST 25),Zusanli(ST 36),Sanyinjiao(SP 6),Huantiao(GB 30),Zhongwan(RN 12),Yinlingquan(SP 9),Danzhong(RN 17),Yinlian(LR 11)and Xuehai(SP 10)in DUB patients was significantly lower than that in healthy people(P<0.01).The average tenderness threshold of each meridian of DUB patients was ranked from high to low:Du meridian>three yang meridians of foot>and Ren meridian>three yin meridians of foot.The tenderness threshold of forced-sensitive acupoint on the three yin meridians of foot was significantly lower than that on the three yin meridians of foot and the du meridian(P<0.05),the tenderne
