基于津液代谢理论研究舒肺清胃饮对原发性干燥综合征患者Th17/Treg细胞的调节作用

摘　　要：目的:观察中药复方舒肺清胃饮治疗原发性干燥综合征(pSS)的临床疗效以及对Th17/Treg细胞的调节作用。方法:选择2018年1月至2019年12月在河南中医药大学第一附属医院风湿病科就诊的22例pSS患者和10名健康志愿者,使用随机数表法将22例pSS患者随机分为对照组和试验组,每组11例。对照组采用硫酸羟氯喹片治疗,试验组采用舒肺清胃饮治疗,两组疗程均为3个月。采用免疫比浊法检测治疗前后患者外周血免疫球蛋白(IgG、IgA、IgM)水平,流式细胞术测定治疗前后外周血Th17和Treg细胞比例,酶联免疫吸附试验检测血清中白细胞介素(IL)-17和IL-10水平。结果:对照组治疗后(对照B组)IgG的浓度显著减少(P<0.01),而IgM和IgA的浓度没有明显变化。试验组治疗后(试验B组)IgG、IgM和IgA的浓度均显著减少(P<0.01)。与健康组比较,pSS患者Th17细胞比例上调(P<0.01),治疗后试验组与对照组患者的Th17细胞比例均见降低(P<0.05),且试验组较对照组显著降低(P<0.05),且试验组还增加了Treg细胞比例(P<0.05)。pSS患者血清中IL-17水平显著高于健康组(P<0.01),治疗后试验组和对照组均见IL-17水平减少(P<0.01),且试验组较对照组显著降低(P<0.01)。同时,治疗后试验组IL-10表达可见上调(P<0.05)。结论:舒肺清胃饮可以调节pSS患者的Th17/Treg免疫平衡,改善炎症水平。Objective:To investigate the clinical efficacy of Shufei Qingwei Decoction on primary Sjogren's syndrome(pSS)and effcet in regulating balance of Th17/Treg cells based on the theory of fluid metabolism.Methods:In this study,22 patients with pSS admitted in the Rheumatology Department of First Affliated Hospital of Henan University of CM from January 2018 to December 2019 and 10 healthy volunteers were recruited.22 psSS patients were randomly divided into two groups:control group(11 cases)and experimental group(1l cases).The control group was treated with hydroxychloroquine sulfate tablets,while the experimental group was orally given Shufei Qingwei Decoction.The treatment course was three months for both groups.Before and after treatment,immunoturbidimetric assay was used to detect the concentration of peripheral blood immunoglobulin(IgA,IgG,and IgM),flow cytometry was used to determine the percentage of Th17 and Treg cells,and enzyme-linked immunosorbent assay was used to detect the concentration of interleukin 17(IL-17)and interleukin 10(IL-10)in serum.Results:After treatment in the control group(control group B),the concentration of IgG decreased significantly(P<0.01),but the concentrations of IgM and IgA did not change significantly.The concentrations of IgG,IgM and IgA in the experimental group decreased significantly after treatment(group B)(P<0.01).Compared with the healthy group,the proportion of Th17 cells in pSS patients increased significantly(P<0.01),and the proportion of Th17 cells in the experimental group and the control group decreased significantly after treatment(P<0.05),and the experimental group was significantly lower than the control group(P<0.05),and the proportion of Treg cells in the experimental group also increased significantly(P<0.05).The serum level of IL-17 in pSS patients was significantly higher than that in the healthy group(P<0.01).Ater treatment,the level of IL-17 decreased significantly in both groups(P<0.01),and the experimental group was significantly lower than the control g
