2010-2020年我国计划生育手术情况分析

摘　　要：目的 了解我国2010-2020年计划生育技术服务情况,为相关部门制定干预措施提供依据。方法 收集2010-2020年我国计划生育手术例数资料,采用SPSS 25.0软件分析我国计划生育手术情况变化情况。结果 2010-2020年我国计划生育手术总例数有所下降,从2010年的22157408例下降为2020年的14687097例,降幅为33.71%。2010-2020年我国放置节育器占比、输精管结扎占比及输卵管结扎占比均呈现逐年下降趋势(P<0.001),取出节育器占比和人工流产占比均呈现逐年上升趋势(P<0.001)。2020年我国东、中、西部地区放置节育器、取出节育器、输卵管结扎、人工流产等手术情况均差异有统计学意义(P<0.001),但输精管结扎手术情况各地区差异无统计学意义(P=0.009)。2020年我国放置节育器、取出节育器、输精管结扎、输卵管结扎、人工流产手术并发症分别为180例、108例、3例、42例、12350例,共计12683例,其中西部地区以上手术并发症发生率均在首位。2020年我国人工流产导致的手术并发症最多,其中人流不全发生率高达128.08/10万。结论 在我国“三孩政策”下,长效避孕计划生育手术量在下降,人工流产手术量在上升,应进一步加强生殖健康宣传,降低非意愿妊娠导致的人工流产数量。Objective To analyze the situation of family planning technical services in China from 2010 to 2020,with providing intervention measures for relevant departments.Methods The number of cases of family planning surguries from 2010 to 2020 was collected to analyze the changes of family planning surguries by using SPSS 25.0 software.Results The total cases of family planning surgery in China decreased from 22157408 in 2010 to 14687097 in 2020,decreased by 33.71%.The proportion of IUD placement,vasectomy and tubal ligation in China showed a downward trend(P<0.001),while the pro-portion of IUD removal and abortion showed an upward trend from 2010 to 2020(P<0.001).There were statistically signifi-cant differences in the operation of placing,removing,tubal ligation and artificial abortion in different areas of China in 2020(P<0.001),but there was no significant difference in vasectomy among all regions(P=0.009).In 2020,there were 12683 cases of complications of placing IUDs(180 cases),taking out IUDs(108 cases),vasectomy(3 cases),tubal ligation(42 cases)and artificial abortion(12350 cases)in China,the incidence of all surgical complications was the first in western China.In 2020,artificial abortion caused the most surgical complications,among which the incidence of incomplete abortion was as high as 12808/100000.Conclusion In the situation of China’s“three-child policy”,the rate of long-acting contraception and family planning surgery was on the decline,while the rate of induced abortions was on the rise.It is necessary to strengthen repro-ductive health publicity to reduce the number of induced abortions caused by unwanted pregnancy.
