针药并治阴虚火旺型更年期失眠临床研究

摘　　要：目的探讨加味酸枣仁汤联合浅针治疗阴虚火旺型更年期失眠的临床疗效。方法选择2020年6月—2022年8月就诊的该型失眠患者80例为失眠组,更年期不失眠者40例作为对照组,依据随机对照法将失眠组患者分为中药组和针药组各40例。中药组予加味酸枣仁汤口服治疗,针药组予加味酸枣仁汤联合浅针治疗,2组均治疗4周。比较2组治疗前后各项临床指标。结果针药组总有效率高于中药组(P<0.05);针药组治疗后中医证候积分、改良Kupperman评分、匹兹堡睡眠质量指数(PSQI)评分均低于中药组(P<0.05);针药组治疗后血清5⁃羟色胺(5⁃HT)和腺苷水平均高于中药组(P<0.05)。结论针药联合治疗阴虚火旺型更年期失眠效果更佳,值得临床推广。Objective To investigate the clinical effect of Suanzaoren decoction combined with shallow needling in the treatment of menopausal insomnia of hyperactivity of fire due to yin deficiency type.Methods A total of 80 patients with menopausal insomnia from June 2020 to August 2022 were selected as insomnia group,and 40 patients without insomnia during menopause were selected as the control group.According to the randomized control method,the insomnia group was divided into Chinese medicine group and combined group,with 40 cases in each.The Chinese medicine group was given Suanzaoren decoction,and the needle medicine group was treated with Chinese medicine combined with shallow needle for 4 weeks.The clinical indexes of the two groups were compared before and after treatment.Results The total effective rate of combined group was higher than that of Chinese medicine group(P<0.05).After treatment,traditional Chinese medicine syndrome score,modified Kupperman score and Pittsburgh sleep quality index(PSQI)score in combined group were lower than those in Chinese medicine group(P<0.05).Serum levels of 5⁃hydroxytryptamine(5⁃HT)and adenosine in combined group were higher than those in Chinese medicine after treatment(P<0.05).Conclusion The combination of acupuncture and medicine in the treatment of climacteric insomnia of hyperactivity of fire due to yin deficiency type has better effect,and is worthy of promotion.
