针灸治疗盆腔炎性疾病后遗症慢性盆腔痛的选穴规律分析

摘　　要：目的采用数据挖掘技术分析针灸治疗盆腔炎性疾病后遗症慢性盆腔痛的取穴规律。方法计算机检索中国期刊全文数据库(CNKI)、万方数据知识服务平台(Wanfang)、维普资讯中文期刊服务平台(VIP)、PubMed数据库针灸治疗盆腔炎性疾病慢性盆腔痛的临床研究文献。建立数据库,进行腧穴频次与归经分析,对腧穴进行关联规则、核心腧穴网络分析及聚类分析。结果共纳入有效文献162篇,涉及84个腧穴;高频应用腧穴分别是关元、三阴交、中极、气海等;应用频次较高的经脉分别为任脉、足太阴脾经等;常用的腧穴组合为三阴交、中极;关联规则分析(置信度≥80%,支持度≥10%)共得到41个关联规则,支持度最高的组穴为“三阴交→足三里”(支持度为42.59%),置信度最高的组穴为“三阴交,阴陵泉→气海”(置信度为94.44%),“中极,阴陵泉→归来”(置信度为94.44%);腧穴可分为3个有效聚类群。结论针灸治疗盆腔炎性疾病后遗症慢性盆腔痛选穴遵循一定规律,可为现代临床研究及治疗提供依据。Objective Using data mining technology,to analyze the rule of acupoint selection in treating chronic pelvic pain caused by sequelae of pelvic inflamatory disease with acupuncture and moxibustion.Methods The clinical research literature on acupuncture and moxibustion in the treatment of chronic pelvic pain of pelvic inflammatory disease was searched by computer from China Journal Full-text Database(CNKI),Wangfang Data Knowledge Service Platform(Wangfang),VIP Information Chinese Journal Service Platform(VIP)and PubMed database.A database was established,frequency of acupoints and meridian analysis were carried out,association rules,core acupoint network analysis and cluster analysis were carried out on acupoints.Results A total of 162 valid literatures were included,involving 84 acupoints.The high-frequency applied acupoints are Guanyuan(RN4),Sanyinjiao(SP6),Zhongji(RN3),Qihai(RN6),etc.The meridians with high frequency of application are ren meridian,foot taiyin spleen meridian,etc.The commonly used combination of acupoints is Sanyinjiao(SP6)and Zhongji(RN3).Association rule analysis(confidence level≥80%,support level≥10%)yielded a total of 41 association rules.The group with the highest support was Sanyinjiao(SP6)→Zusanli(ST36)(support level 42.59%),the group with the highest confidence was Sanyinjiao(SP6),Yinlingquan(SP9)→Qihai(RN6)(confidence level 94.44%),and Zhongji(RN3)and Yinlingquan(SP9)→Guilai(ST29)(confidence level 94.44%).Acupoints can be divided into three effective cluster groups.Conclusion Acupuncture and moxibustion in the treatment of chronic pelvic pain after pelvic inflammatory disease follows certain rules,which can provide basis for modern clinical research and treatment.
