中药汤剂联合常规西药治疗中青年高血压病Meta分析

摘　　要：目的:系统评价中药汤剂治疗中青年高血压病的临床疗效。方法:利用维普数据库、万方数据库、中国生物医学文献数据库和中国知网数据库,检索建库至2021年9月公开发表的关于中药汤剂治疗中青年高血压病的随机对照试验研究;对纳入文献进行方法学质量评估;使用RevMan 5.3软件进行统计学分析。结果:最终纳入研究12项,共涉及患者828例,其中治疗组(予常规西药联合中药汤剂治疗)417例,对照组(予单纯西药治疗)411例。Meta分析结果显示,在常规西药治疗基础上联合中药汤剂能够进一步降低中青年高血压病患者收缩压、舒张压、中医证候积分、血管紧张素II水平、血醛固酮水平,提高临床疗效,且未见明显不良反应。在调节血肾素水平方面,常规西药联合中药汤剂治疗与单纯西药治疗的差异无统计学意义。结论:现有证据表明,中药汤剂联合常规西药治疗中青年高血压病的疗效优于单纯西药治疗,且未见明显不良反应,但仍需更多多中心、高质量的临床研究加以证实。Objective:To systematically evaluate the clinical efficacy of TCM decoction on hypertension in young and middleaged people.Methods:Through VIP database,Wanfang data,SinoMed,and CNKI databases,the studies of randomized controlled trials on treating hypertension in young and middle-aged people with TCM decoction published from the establishment of each database to September 2021 were selected.Methodological quality evaluation of the included literature was performed,and a statistical analysis was conducted by RevMan 5.3 software.Results:Totally 12 studies were included in this study,involving a total of 828 cases,including 417 cases in the treatment group(took TCM decoction plus conventional Western medicine)and 411 cases in the control group(took conventional western medicine only).The Meta-analysis results showed that the combination of TCM decoction and conventional western medicine can further reduce systolic blood pressure,diastolic blood pressure,TCM syndrome score,angiotensin II level,and blood aldosterone level in the young and middle-aged people with hypertension.The clinical efficacy was improved,and no obvious adverse reaction was found.In terms of regulating the level of blood plasma renin,there was no statistically significant difference between conventional western medicine plus TCM decoction and simple western medicine.Conclusion:The existing evidence shows that the curative effect of TCM decoction plus conventional western medicine on hypertension in young and middle-aged people is better than that of western medicine alone,and there is no obvious adverse reaction.Its conclusion still needs to be confirmed by more multi-center and high-quality clinical studies.
