摘　　要：目的评价他汀类药物对于慢性肾脏病患者动静脉内瘘通畅率的影响。方法限定时间为从建库开始截至2022年7月9日,对PubMed、Embase、Web of Science、Cochrane Library、万方、知网、维普、中国生物医学文献数据库进行系统的文献检索,搜集他汀类药物对于动静脉内瘘通畅率的随机对照试验和队列研究,采用RevMan 5.3软件进行Meta分析。结果共纳入3篇随机对照试验和5篇队列研究,共纳入自体动静脉内瘘(arteriovenous fistula,AVF)患者19755例,移植物动静脉内瘘(arteriovenous graft,AVG)3476例。分析结果显示:(1)他汀类药物对AVF通畅率显著优于对照组(RR=1.09,95%CI 1.01~1.18,P=0.03),进一步的亚组分析显示,仅有亚洲地区的AVF通畅率显著优于对照组(RR=1.19,95%CI 1.03~1.38,P=0.02)。(2)他汀类药物对AVG通畅率较对照组差异无统计学意义(RR=1.00,95%CI 0.87~1.15,P=1.00)。结论他汀类药物可能有助于提高慢性肾脏病患者,尤其是亚洲地区的AVF通畅率。Objective To evaluate the effect of statins on the patency of arteriovenous fistula in patients with chronic renal disease.Methods Up to July 9,2022,a systematic literature search was conducted in PubMed,Embase,Web of Science,Cochrane Library,Wanfang,CNKI,VIP,and Chinese Biomedical Literature database to collect randomized controlled trials and cohort studies on the patency rate of statins to arteriovenous fistulas.RevMan5.3 software was used for Meta-analysis.Results A total of 3 randomized controlled trials and 5 cohort studies were included,including 19755 patients with autologous arteriovenous fistula(AVF)and 3476 patients with arteriovenous graft(AVG).The results showed that:(1)The patency rate of statins to AVF was significantly better than that of the control group(RR=1.09,95%CI 1.01~1.18,P=0.03).Further subgroup analysis showed that only the patency rate of AVF in Asia was significantly better than that of the control group(RR=1.19,95%CI 1.03~1.38,P=0.02).(2)There was no significant difference in the patency rate of statins to AVG compared with the control group(RR=1.00,95%CI 0.87~1.15,P=1.00).Conclusions Statins may help improve the patency of AVF in patients with chronic kidney disease,especially in Asia.
