耳穴压豆联合腹部按摩对抗精神病药物所致便秘患者排便情况及生活质量的影响

摘　　要：目的探究耳穴压豆联合腹部按摩对抗精神病药物所致便秘患者排便情况及生活质量的影响。方法回顾性分析2019年10月至2021年10月在苏州市广济医院接受治疗的100例抗精神病药物所致便秘患者的临床资料,根据护理方式的不同进行分组,分别为对照组和观察组,各50例。对照组按照常规护理方法进行干预,观察组在对照组护理方法的基础上采用耳穴压豆联合腹部按摩进行干预。比较2组患者的排便情况和生活质量等。结果干预2周和4周后,2组的Wexner便秘评分表评分均较干预前降低;2组的血清胃动素和胃泌素水平均高于干预前;2组的肠道中的大肠埃希菌和葡萄球菌数量均减少,双歧杆菌和乳酸菌数量均增多;2组的健康调查量表36评分均升高,且观察组的上述指标均较同期对照组更优,组间和组内差异均有统计学意义(P均<0.05)。观察组的腹胀、便质干结和排便困难情况的缓解时间均较对照组更快(P均<0.05)。结论采用耳穴压豆联合腹部按摩对抗精神病药物所致便秘患者进行干预,能够缓解其便秘症状,促进排便,调节胃肠激素水平,改善肠道菌群失调,提高生活质量。Objective To explore the effect of auricular acupoint pressing with beans combined with abdominal massage on the defecation and quality of life of patients with antipsychotic-induced constipation.Methods A retrospective analysis was made on the clinical data of 100 patients with antipsychotic-induced constipation at Suzhou Guangji Hospital from October 2019 to October 2021.According to the different nursing methods,they were assigned to control and observation groups,each with 50 patients.The control group received intervention of routine nursing method,while the observation group received intervention of auricular acupoint pressing with beans combined with abdominal massage on the basis of the control group.The defecation status and quality of life of the two groups were compared.Results After 2 and 4 weeks of intervention,the Wexner Constipation Scale scores of both groups were lower than those before intervention.The levels of serum motilin and gastrin were higher than those before intervention.The number of Escherichia coli and Staphylococcus in the intestinal tract were reduced,and the number of Bifidobacterium and lactic acid bacteria were increased.The scores of 36-Item Short-Form Health Survey were increased.The above indicators in the observation group were better than those in the control group on the corresponding period.There were statistically significant differences between and within groups(all P<0.05).The relief time of abdominal distension,dry stool,and difficulty in defecation in the observation group was shorter than that in the control group(all P<0.05).Conclusion The intervention of auricular acupoint pressing with beans combined with abdominal massage for patients with antipsychotic-induced constipation can alleviate the constipation symptoms,promote the defecation,regulate the gastrointestinal hormone levels,improve the imbalance of gut microbiota,and improve the quality of life.
