iLESSYS Delta和显微镜下手术治疗腰椎管狭窄症疗效分析

摘　　要：目的探讨Delta大通道内窥镜系统(iLESSYS Delta)和显微镜下手术治疗腰椎管狭窄症(lumbar spinal stenosis,LSS)的疗效。方法选择2019年3月~2020年3月收治的72例LSS患者为研究对象,以简单随机数字表法分为研究组及对照组各36例。研究组患者接受iLESSYS Delta技术治疗,对照组患者接受显微镜下手术治疗。对比两组患者手术相关指标、临床疗效。结果研究组患者切口长度、术中出血量均明显小于对照组患者,而手术时间明显长于对照组患者(P<0.05)。术后1个月,两组患者VAS评分、ODI指数均显著低于术前,且研究组<对照组(P<0.05);而JOA评分均显著高于术前,研究组>对照组(P<0.05)。术后3个月,两组患者ODI指数均显著低于术前,且研究组<对照组(P<0.05),JOA评分均显著高于术前,且研究组>对照组(P<0.05);末次随访时,两组患者的VAS评分、ODI指数和JOA评分无显著性差异(P>0.05)。末次随访时,两组患者临床疗效优良率(91.67%vs 88.89%)无显著性差异(P>0.05)。结论iLESSYS Delta和显微镜下手术均是治疗LSS的安全、有效术式。Objective To discuss the efficacy of Delta large channel endoscopy system(iLESSYS Delta)and microscopic surgery in the treatment of lumbar spinal stenosis(LSS).Methods Seventy-two patients with LSS admitted to our hospital from March 2019 to March 2020 were selected as the research objectsand randomly divided into the study group and the control group with a simple random number table method,with 36 cases in each.Patients in the study group received iLESSYS Delta technology treatment,and those in the control group received surgery under the microscope.The surgical related indicators and clinical efficacy between two groups of patients were compared.Results The incision length and intraoperative blood loss in the study group were significantly less than those in the control group,while the operation time was significantly longer than that in the control group(P<0.05).One month after surgery,the VAS score and ODI index of both groups of patients were significantly lower than thosebefore surgery,and VAS score and ODI indexof the study group were lower than those of the control group(P<0.05).The JOA scores were significantly higher than those before surgery,with the study group being higher than those of the control group(P<0.05).Three months after surgery,the ODI index of both groups of patients was significantly lower than thatbefore surgery,and the ODI indexof the study group was lower than that of the control group(P<0.05).The JOA score was significantly higher than that before surgery,and the JOA score ofthe study group was higher than that of the control group(P<0.05).At the last follow-up,there was no significant difference in VAS score,ODI index,and JOA score between the two groups of patients(P>0.05).At the last follow-up,there was no significant difference in the clinical efficacy between the two groups of patients(91.67%vs 88.89%)(P>0.05).Conclusion Both iLESSYS Delta and microscopic surgery are safe and effective procedure for LSS.
