摘　　要：目的:系统评价白虎加桂枝汤治疗类风湿性关节炎的临床疗效及安全性。方法:通过电子计算机搜索中国知网、万方、维普、PubMed数据库及中国生物医学文献数据库,文献筛选从建库至2022年7月27日中白虎加桂枝汤治疗类风湿性关节炎RCTs-临床随机对照试验。由两名独立研究者对文献进行筛选及数据提取,使用Cochrane系统评价手册对纳入的文献进行偏倚风险质量评价,利用Revman5.3软件进行Meta分析。结果:纳入13篇临床随机对照试验文献,共1114名患者,包括观察组579名患者,对照组535名患者。Meta分析结果显示,白虎加桂枝汤治疗类风湿性关节炎的临床效果显著。总体有效率OR=4.72[3.10,7.18],P<0.000 01(治愈率OR=1.77[1.24,2.53],P=0.002);中医证候积分MD=-1.15[-1.38,-0.91],P<0.000 01;炎症指标MD=-6.32[-8.21,-4.42],P<0.000 1;DAS28评分MD=-0.46[-0.68,-0.23],P<0.000 1;副作用OR=0.37[0.19,0.75],P=0.005。结论:经研究发现,白虎加桂枝汤治疗类风湿性关节炎临床疗效确切,但所筛选文献整体质量水平不高,存在偏倚风险。Objective:To systematically evaluate the clinical efficacy and safety of Baihu Jiaguizhi decoction in the treatment of rheumatoid arthritis.Methods:By searching the databases of CNKI,Wanfang,Vip,PubMed,and Chinese Biomedical Literature Database through electronic computers,literature screening was conducted from the establishment of the database to the clinical randomized controlled trial of Baihu Jiaguizhi Tang in the treatment of rheumatoid arthritis RCTs on July 27,2022.Literature screening and data extraction were conducted through two independent researchers.Use the Cochrane system evaluation manual to conduct a biased risk quality evaluation of the included literature,and use Revman 5.3 software for meta-analysis.Results:13 clinical randomized controlled trials were included.There are a total of1114 patients,including 579 patients in the observation group and 535 patients in the control group.Meta-analysis results showed that Baihu Jiaguizhi decoction has significant clinical effect in the treatment of rheumatoid arthritis:the overall effective rate OR=4.72[3.10,7.18],P<0.00001(the cure rate OR=1.77[1.24,2.53],P-0.002);TCM syndrome score MD=-1.15[-1.38,-0.91],P<0.00001;Inflammatory index MD--6.32[-8.21,-4.42],P<0.001;DAS28 score MD--0.46[-0.68,-0.23],P<0.001;side effects OR=0.37[0.19,0.75],P-0.005.Conclusion:After research,it was found that the clinical efficacy of Baihu Jiaguizhi decoction in the treatment of rheumatoid arthritis is definite,but the overall quality of the selected literature is not high,and there isarisk ofbias.
