家庭医生签约服务模式在社区高血压患者中的管理效果分析

摘　　要：目的:分析应用家庭医生签约服务模式对社区高血压患者进行管理的效果。方法:选取2020年1月—2021年1月凯里市西门街道社区卫生服务中心收治的100例高血压患者作为研究对象,随机分为对照组(n=30)和观察组(n=70)。对照组采用常规社区慢性病管理模式,观察组采用家庭医生签约服务管理模式。比较两组血压、血糖、血脂指标、体重指数、自我管理能力、健康管理依从性评分。结果:管理后,观察组收缩压、舒张压、空腹血糖、甘油三酯、低密度脂蛋白胆固醇、体重指数低于对照组,高密度脂蛋白胆固醇高于对照组,差异有统计学意义(P<0.05)。管理后,观察组科学锻炼率、科学服药率、监测血压率、规律复查率均高于对照组,差异有统计学意义(P<0.05)。管理后,观察组健康管理依从性评分高于对照组,差异有统计学意义(P=0.006)。结论:家庭医生签约服务模式应用于社区高血压患者的效果优良,能够有效控制患者的血压水平,降低体重指数,改善血脂水平,提高患者自我管理能力及健康管理依从性。Objective:To analyze the management effect of applying the family doctor contract service model in the hypertension patients in the community.Methods:A total of 100 patients with hypertension in Kaili City Ximen Street Community Health Service Center from January 2020 to January 2021 were selected as the study subjects.They were randomly divided into control group(n=30)and observation group(n=70).The control group adopted the conventional community chronic disease management model,and the observation group adopted the family doctor contract service management model.The blood pressure,blood glucose,blood lipid index,body mass index,self-management ability,and health management compliance score between the two groups were compared.Results:After administration,systolic blood pressure,diastolic blood pressure,fasting blood glucose,triglycerides,low density lipoprotein cholesterol and body mass index in the observation group were lower than those in the control group,and high density lipoprotein cholesterol in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After management,the scientific exercise rate,scientific medication rate,monitored blood pressure rate and regular review rate in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After management,the health management compliance score in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.006).Conclusion:The family doctor contract service model has excellent effect in patients with hypertension in the community,which can effectively control the blood pressure level,reduce body mass index,improve blood lipid level,improve patients'self-management ability and health management compliance.
