摘　　要：痔是我国最常见的肛周疾病,内外痔的形成机制各有不同。根据“肛垫下移学说”提出的痔上黏膜环切术(PPH)至今已发展20余年,对环状脱垂的Ⅲ、Ⅳ度内痔、直肠脱垂等疗效较好,但传统PPH术采用单荷包缝合单牵引线的方法,术中切除黏膜宽度较窄且均匀度差,术后复发率高。为改善上述不足,学者从单双荷包的选择、选择性分段缝合、降落伞缝合及荷包缝合的针数、高度及进针深度等方面作出改良,以求病人术中切除效果明显,手术并发症少、术后复发率低。Hemorrhoids are the most frequent perianal diseases in China.There are many differences in the formation mechanisms of internal and external hemorrhoids.Put forward according to the sliding anal cushion theory,the procedure for prolapse and hemorrhoids(PPH)has been developed for more than 20 years,which has good efficacy onⅢ,Ⅳdegree internal hemorrhoids and rectal prolapse,but conservative PPH uses a single purse-string suture with one pull line,so that the excised mucosa has a narrow width and poor uniformity,thus leading to a high postoperative recurrence rate.In order to improve those shortcomings,surgeons have made many attempts and improvements,including the selection of single or double purse strings,selective segmental sutures,parachute sutures,the number of needles,stitch height and depth of needle insertion in purse-string suture,so as to achieve obvious surgical resection results with fewer surgical complications and lower postoperative recurrence rate.
