摘  要:脓毒症是宿主对感染的失调反应而引起的危及生命的器官功能障碍,目前沿用脓毒症3.0定义。儿童脓毒症是一种具有高发病率、高致残率和高死亡率的儿童临床常见危重症,随着病情加重可进展为脓毒性休克、多器官功能障碍等。该综述简述了脓毒症的定义以及器官功能障碍、机体免疫功能和细胞功能障碍、机体代谢变化、机体微循环变化等病理生理变化,并概括相关的儿童脓毒症早期诊断生物学标志物,为儿童脓毒症临床诊治策略制定、病情评估、病程监测和预后判断提供思路。Sepsis is a life-threatening organ dysfunction caused by the host's dysfunctional response to infection,currently defined as sepsis 3.0.Pediatric sepsis is a common clinical critical disease with high incidence rate,high disability rate and high mortality.With the aggravation of the disease,it can progress to septic shock,multiple organ dysfunction,etc.This review outlines the definition of sepsis and pathological and physiological changes such as organ dysfunction,immune and cellular dysfunction,metabolic changes,and microcirculation changes,and summarizes the relevant biomarkers for early diagnosis of pediatric sepsis,provides ideas for clinical diagnosis and treatment strategies,condition evaluation,disease course monitoring,and prognosis judgment of pediatric sepsis.
