分期辨证针刺疗法联合丹红注射液对脑卒中偏瘫患者生活能力及肢体运动功能的影响

摘　　要：目的探讨分期辨证针刺疗法联合丹红注射液对脑卒中偏瘫患者生活能力及肢体运动功能的影响。方法选取2020年1月至2022年7月至上饶市中医院就诊的82例缺血性脑卒中偏瘫患者作为研究对象,按随机数字表法分为观察组与对照组,各41例。对照组在常规治疗的基础上,加用丹红注射液。观察组在对照组的基础上,予分期辨证针刺疗法。比较两组患者治疗前后的神经功能受损情况、生活能力、肢体运动功能与血液流变学指标。结果治疗前,两组患者的美国国立卫生研究院卒中量表(NIHSS)、改良巴氏指数(MBI)、改良Rankin量表(mRS)评分、血液流变学指标比较,差异无统计学意义(P>0.05);治疗后,两组患者的MBI评分高于本组治疗前,NIHSS评分、mRS评分、高切全血黏度、低切全血黏度、血浆黏度、红细胞压积、血小板黏附率低于本组治疗前,且观察组的MBI评分高于对照组,NIHSS评分、mRS评分、高切全血黏度、低切全血黏度、血浆黏度、红细胞压积、血小板黏附率低于对照组,差异有统计学意义(P<0.05)。结论分期辨证针刺疗法联合丹红注射液治疗脑卒中偏瘫患者,可有效提高日常生活能力,改善肢体神经功能受损情况和运动功能。Objective To explore the effects of staged dialectical acupuncture therapy combined with Danhong Injection on activities of daily living and limb motor function in patients with stroke hemiplegia.Methods A total of 82 patients with ischemic stroke hemiplegia who went to Shangrao Traditional Chinese Medicine Hospital from January 2020 to July 2022 were selected as research subjects,they were divided into observation group and control group according to random number table method,with 41 cases in each group.On the basis of routine treatment,the control group was supplemented with Danhong Injection.On the basis of the control group,the observation group was treated with staged dialectical acupuncture therapy.The neurological impairment,activities of daily living,limb motor function and hemorheology indexes of the two groups were compared before and after treatment.Results Before treatment,there were no statistically significant differences in the National Institutes of Health stroke scale(NIHSS),modified Barthel index(MBI),modified Rankin score(mRS)and hemorheology indexes between the two groups(P>0.05).After treatment,the MBI scores of both groups of patients were higher than those before treatment,while the NIHSS score,mRS score,high shear whole blood viscosity,low shear whole blood viscosity,plasma viscosity,hematocrit,and platelet adhesion rate were lower than those before treatment,the MBI score of the observation group was higher than that of the control group,while the NIHSS score,mRS score,high shear whole blood viscosity,low shear whole blood viscosity,plasma viscosity,hematocrit,and platelet adhesion rate of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).Conclusion Staged dialectical acupuncture therapy combined with Danhong Injection can effectively improve activities of daily living,neurological impairment and motor function of limbs in patients with stroke hemiplegia.
