摘　　要：目的分析脑脊液乳酸(LA)、天门冬氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)联合检测在婴幼儿脑膜炎中的诊断效能。方法选取2018年4月至2022年4月新余市人民医院收治的64例脑膜炎患儿为研究对象,按脑脊液常规及生化结果将其分为细菌性脑膜炎组(18例)与非细菌性脑膜炎组(46例),检测并比较两组脑脊液内LA、AST、LDH差异;另外绘制ROC曲线,分析脑脊液内LA、AST、LDH诊断细菌性脑膜炎的效能。结果细菌性脑膜炎组脑脊液LA、AST、LDH高于非细菌性脑膜炎组,差异有统计学意义(P<0.05)。ROC结果显示,LA、AST、LDH联合检测诊断细菌性脑膜炎的AUC高于三者单独检测。结论LA、AST、LDH在细菌性脑膜炎患儿脑脊液内呈高表达,三者联合可有效诊断细菌性脑膜炎,为临床治疗提供可靠依据,临床应用价值较高,值得进行大力推行。Objective To analyze the diagnostic efficacy of combined detection of cerebrospinal fluid lactic acid(LA),aspartate aminotransferase(AST),and lactate dehydrogenase(LDH)in infants with meningitis.Methods A total of 64 children with meningitis admitted to Xinyu People's Hospital from April 2018 to April 2022 were selected as research subjects.They were divided into bacterial meningitis group(18 cases)and non bacterial meningitis group(46 cases)based on routine and biochemical results of cerebrospinal fluid.The differences in LA,AST,and LDH in cerebrospinal fluid between the two groups were detected and compared.In addition,ROC was drawn to analyze the efficacy of LA,AST,and LDH in cerebrospinal fluid in diagnosing bacterial meningitis.Results The levels of LA,AST,and LDH in cerebrospinal fluid in bacterial meningitis group were significantly higher than those in non bacterial meningitis group(P<0.05).The ROC results showed that the combined detection of LA,AST,and LDH for the diagnosis of bacterial meningitis had a higher AUC than the single detection of the three.Conclusion LA,AST,and LDH are highly expressed in cerebrospinal fluid of children with bacterial meningitis.The combination of the three can effectively diagnose bacterial meningitis,provide reliable evidence for clinical treatment,and have high clinical application value,which is worth vigorously promoting.
