摘  要:目的研究CT扫描不同图像后处理技术对主动脉夹层破口检出率的影响。方法选取2019年6月至2022年8月肇庆市第一人民医院收治的80例以突发性胸痛为主诉的主动脉夹层患者作为研究对象,以MRI诊断为金标准,行多层螺旋CT血管成像检查,采用容积重建(VR)、多平面重建(MPR)、曲面重建(CPR)等技术进行图像后处理。结果有74例明确显示夹层破口,检出率为92.50%。根据DeBakey分型,Ⅰ型36例(45.00%),Ⅱ型14例(17.50%),Ⅲ型30例(37.50%)。80例患者的多层螺旋CT后处理图像效果良好,原始横轴图像能清晰显示隔膜及真假腔,其中31例(38.54%)显示夹层破口,VR显示夹层破口69例(86.25%),MPR显示夹层破口71例(88.75%),CPR显示夹层破口50例(62.50%)。MPR和VR对夹层破口的总检出率高于CPR,差异有统计学意义(P<0.01)。MPR和VR对Ⅰ型、Ⅱ型的准确性高于CPR,差异有统计学意义(P<0.01)。CPR和MPR对Ⅲ型的准确性高于VR,差异有统计学意义(P<0.01)。结论主动脉夹层破口采用CT不同图像后处理技术各有优缺点,其中MPR操作简单、对夹层破口的显示能力强及准确分型,可作为首选。Objective To study the effect of different CT image post-processing techniques on the detection rate of aortic dissection.Methods A total of 80 cases of aortic dissection with sudden chest pain as the chief complaint were selected from Zhaoqing First People's Hospital from June 2019 to August 2022.Multi-slice spiral CT angiography was performed with MRI as the gold standard,and the images were post-processed by volume reconstruction(VR),multi-plane reconstruction(MPR),surface reconstruction(CPR)and other techniques.Results There were 74 cases of sandwich rupture,the detection rate was 92.50%.According to DeBakey classification,there were 36 cases of typeⅠ(45.00%),14 cases of typeⅡ(17.50%)and 30 cases of typeⅢ(37.50%).The multi-slice spiral CT post-processing images of 80 patients had good results,and the original transverse axis images could clearly show the diaphragm and true and false cavity,of which 31 cases(38.54%)showed the sandwich rupture,69 cases(86.25%)showed the sandwich rupture in VR,71 cases(88.75%)showed the sandwich rupture in MPR,and 50 cases(62.50%)showed the sandwich rupture in CPR.The total detection rate of MPR and VR was higher than that of CPR,and the difference was statistically significant(P<0.01).The accuracy of MPR and VR for typeⅠand typeⅡwas higher than that of CPR,and the difference was statistically significant(P<0.01).The accuracy of CPR and MPR for typeⅢwas higher than VR,and the difference was statistically significant(P<0.01).Conclusion Different CT image post-processing techniques for aortic dissection have advantages and disadvantages,among which MPR can be used as the first choice because of its simple operation,strong display ability for dissection and accurate classification.
