摘 要:高血压和骨代谢在生理调节上有共同点,特定种类的降压药也可能影响骨密度或降低骨质疏松症相关骨折风险。虽然现有研究不足以明确哪类降压药对骨骼有益,但目前大部分研究支持血管紧张素转换酶抑制剂、血管紧张素受体阻滞剂和β受体拮抗剂能够改善骨密度或降低骨折风险,袢利尿剂则增加骨折风险,而噻嗪类利尿剂和钙通道阻滞剂相关结论互相矛盾。基于高血压的高患病率及降压药的广泛使用,探讨不同种类降压药对骨骼的影响很重要。通过总结近年来关于不同种类降压药与骨质疏松症相关性研究,为易患骨质疏松症高危人群的高血压患者选择合适的降压药提供临床依据。Hypertension and bone metabolism have common point in physiological regulation.Certain kinds of anti-hypertensive drugs may also affect bone mineral density or reduce the risk of osteoporosis related fractures.Although the existing studies are not enough to determine which type of anti-hypertensive drugs are beneficial to the bone,most of the current studies support that angiotensin converting enzyme inhibitors,angiotensin receptor blockers,and beta-blockers improve bone mineral density or reduce the risk of fracture.Loop diuretics increase the risk of fracture.Thiazide diuretics and calcium channel blockers have conflicting conclusion.Based on the high prevalence of hypertension and the widespread use of anti-hypertensive drugs,it is important to explore the effects of different kinds of anti-hypertensive drugs on the bone.By summarizing the research on the correlation between different kinds of anti-hypertensive drugs and osteoporosis in recent years,we provide clinical evidence for selecting appropriate anti-hypertensive drugs for hypertensive patients in high-risk groups prone to osteoporosis.
