逍遥散加味方治疗乳腺增生症的临床研究

摘　　要：目的探讨逍遥散加味方治疗乳腺增生症的疗效及对乳房超声指标、激素水平的影响。方法纳入台州市中心医院(台州学院附属医院)于2019年1月至2022年1月收治的85例肝郁气滞型乳腺增生症患者,根据用药方法的不同分为对照组(n=41)和观察组(n=44),对照组给予他莫昔芬片治疗,观察组给予逍遥散加味方联合他莫昔芬片治疗。比较两组患者的治疗效果及治疗前后的疼痛评分、激素水平[催乳素(prolactin,PRL)、孕酮(progesterone,P)、雌二醇(estradiol,E2)]、乳房超声指标(腺体层厚度和肿块最大直径)及焦虑自评量表(self-rating anxiety scale,SAS)和抑郁自评量表(self-rating depression scale,SDS)评分。结果观察组治疗后总有效率(93.18%)明显高于对照组(78.05%)(P<0.05);观察组治疗后的视觉模拟评分法(visualanaloguescale,VAS)评分、血清中的PRL、E2水平明显低于对照组,血清中P水平明显高于对照组(P<0.05);观察组治疗后的腺体层厚度和肿块最大直径明显小于对照组(P<0.05);观察组患者治疗后的SAS和SDS评分明显低于照组(P<0.05)。结论逍遥散加味方治疗乳腺增生症效果较好,可显著减轻患者乳房疼痛,减小腺体层厚度和肿块最大直径,改善患者负面情绪。Objective To explore the therapeutic effect of Xiaoyao powder modified formula on breast hyperplasia and its impact on breast ultrasound indicators and hormone levels.Methods Totally 85 patients with breast hyperplasia of liver stagnation and Qi stagnation type admitted to Taizhou Central Hospital(Taizhou University Hospital)from January 2019 to January 2022 were divided into control group(n=41)and observation group(n=44)according to the different medication methods.The control group was treated with tamoxifen tablets,and observation group was treated with Xiaoyao powder modified formula plus tamoxifen tablets.The treatment effect,pain scores,hormone levels[prolactin(PRL),progesterone(P),estradiol(E2)],breast ultrasound indicators(thickness of the glandular layer and the largest diameter of the mass),self-rating anxiety scale(SAS)and self-rating depression scale(SDS)scores before and after treatment were compared between the two groups.Results The total effective rate of observation group after treatment(93.18%)was significantly higher than that of control group(78.05%),(P<0.05);The visual analogue scale(VAS)score,serum PRL,E2 levels,and progesterone levels in observation group after treatment were significantly lower than those in control group(P<0.05);The thickness of the glandular layer and the maximum diameter of the tumor in observation group after treatment were significantly smaller than those in control group(P<0.05);The SAS and SDS scores of observation group patients after treatment were significantly lower than those of control group(P<0.05).Conclusion The Xiaoyao powder modified formula has a good effect in the treatment of breast hyperplasia,which can significantly reduce the Breast pain of patients,reduce the thickness of the glandular layer and the maximum diameter of the mass,and improve the negative emotions of patients.
