踝关节骨折合并下胫腓韧带损伤的手术内固定治疗效果分析

摘　　要：目的 分析手术内固定治疗踝关节骨折合并下胫腓韧带损伤的效果。方法 选取2021年5月-2022年4月北京北亚骨科医院治疗的86例踝关节骨折合并下胫腓韧带损伤患者为研究对象,随机数表法分为两组,各43例,其中对照组患者行骨折切开复位内固定联合下胫腓螺钉治疗,观察组在对照组治疗的基础上联合铆钉修复下胫腓韧带治疗。比较两组患者康复情况、患者关节功能、生存质量和疼痛程度评分、临床疗效、二次手术发生率。结果 观察组患者骨折愈合时间和下胫腓韧带损伤修复时间短于对照组,差异有统计学意义(P<0.05);治疗后观察组关节功能评分、生存质量评分均高于对照组,疼痛程度评分低于对照组,差异有统计学意义(P<0.05)。观察组患者总有效率(97.67%)高于对照组(81.40%),差异有统计学意义(χ^(2)=4.468,P<0.05);观察组患者二次手术发生率低于对照组,差异有统计学意义(P<0.05)。结论 手术内固定治疗踝关节骨折合并下胫腓韧带能取得较为显著的疗效,患者的临床症状得到显著改善,促进恢复骨折稳定性和功能关节,提高患者生存质量。Objective To analyze the effect of surgical internal fixation in the treatment of ankle fracture combined with inferior tibiofibular ligament injury.Methods From May 2021 to April 2022,86 patients with ankle fracture com⁃bined with lower tibiofibular ligament injury treated in Beijing Beiya Orthopedic Hospital were selected as the re⁃search objects.They were divided into two groups by random number table method,43 cases in each group.The con⁃trol group received open reduction and internal fixation of fracture combined with lower tibiofibular screw treatment,and the observation group was combined with rivet repair of lower tibiofibular ligament treatment on the basis of the control group.The rehabilitation status,joint function,quality of life,pain degree score,clinical efficacy and second⁃ary surgery incidence were compared between the two groups.Results Fracture healing time and repair time of lower tibiofibular ligament injury in observation group were shorter than those in control group,and the difference was statis⁃tically significant(P<0.05).After treatment,the joint function score and quality of life score of the observation group were higher than those of the control group,and the pain degree score was lower than that of the control group,and the difference was statistically significant(P<0.05).The total effective rate of observation group(97.67%)was higher than that of control group(81.40%),the difference was statistically significant(χ^(2)=4.468,P<0.05).The incidence of second operation in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion The treatment of ankle fracture combined with lower tibiofibular ligament by internal fixation can achieve more significant curative effect.The clinical symptoms of patients are significantly improved,which promotes the recovery of fracture stability and joint function,and improves the quality of life of patients.
