雷珠单抗联合全视网膜光凝治疗增殖期糖尿病性视网膜病变的疗效分析

摘　　要：目的 探究对增殖期糖尿病性视网膜病变患者实施雷珠单抗联合全视网膜光凝治疗的效果。方法 选取2022年2月—2023年2月惠安惠光爱尔眼科医院60例(90眼)增殖期糖尿病性视网膜病变患者。根据患者在院期间接受的治疗方式的差异对其进行分组,30例患者(48眼)纳入对照组接受全视网膜光凝治疗,30例患者(42眼)纳入研究组接受雷珠单抗联合全视网膜光凝治疗。比较两组患者治疗前、治疗后1个月、治疗后3个月的矫正视力、黄斑中心厚度,对比两组患者治疗后3个月的新生血管消退情况,统计两组患者在术后3个月内黄斑水肿以及玻璃体积血的发生情况。结果 两组患者治疗前、治疗后1个月、治疗后3个月3个时间点的矫正视力水平比较,差异无统计学意义(P>0.05)。治疗前两组患者黄斑中心厚度比较,差异无统计学意义(P>0.05);治疗后1个月、治疗后3个月研究组的黄斑中心厚度均低于对照组患者,差异有统计学意义(P<0.05)。治疗后3个月研究组患者的新生血管消退率明显高于对照组患者,差异有统计学意义(P<0.05)。术后3个月内,研究组患者的黄斑水肿、玻璃体积血发生率均低于对照组患者,差异有统计学意义(P<0.05)。结论 对于增殖期糖尿病性视网膜病变患者的治疗方式,雷珠单抗联合全视网膜光凝治疗的效果理想,不仅有利于患者视力的提升,还有利于视网膜解剖结构的改善,促进新生血管消退,综合提高患者的治疗效果。Objective To explore the effect of ranibizumab combined with panretinal photocoagulation in patients with proliferative diabetic retinopathy.Methods 60 proliferative diabetic retinopathy patients and 90 eyes from Hui'an Huiguang Aier Eye Hospital from February 2022 to February 2023 were selected and grouped according to differences in the type of treatment they received while in the hospital.Thirty patients(48 eyes)were included in the control group and received panretinal photocoagulation,and 30 patients(42 eyes)were included in the study group and received ranibizumab combined with panretinal photocoagulation.The corrected visual acuity and central macular thickness were compared between the two groups before treatment,1 month after treatment,and 3 months after treatment.The regression of neovascularization in the two groups of patients 3 months after treatment was compared,and the incidence of macular edema and vitreous hemorrhage in the two groups of patients within 3 months after treatment were counted.Results There was no statistically significant difference in corrected visual acuity between the two groups at the three time points before treatment,1 month after treatment,and 3 months after treatment(P>0.05).Before treatment,there was no statistically significant difference in macular center thickness between the two groups(P>0.05);one month after treatment and three months after treatment,the central macular thickness of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).Three months after treatment,the rate of neovascularization in the study group was significantly higher than that in the control group,and the differ‐ence was statistically significant(P<0.05).Within 3 months after operation,the incidences of macular edema and vitre‐ous hemorrhage in the study group were lower than those in the control group,and the difference was statistically sig‐nificant(P<0.05).Conclusion For the treatment of patients with proliferative diabetic retin
