摘　　要：目的 评估个体化医学营养治疗对妊娠期糖尿病(gestational diabetes mellitus, GDM)患者产后血糖代谢的影响。方法 随机选取将2020年6月-2022年6月在广西北流市人民医院接受常规产前检查并分娩的214例GDM患者分为两组,接受个体化医学营养治疗(individualized medical nutrition therapy, IMNT)的观察组有110例,接受常规妊娠期糖尿病营养指导的对照组有104例。比较两组产后6周的葡萄糖耐量试验结果。结果 观察组产后42 d的FPG、餐后1 h和2 h血糖水平明显低于对照组,差异有统计学意义(P<0.05),观察组产后糖代谢异常的发生率为30.91%,明显低于对照组的45.20%,差异有统计学意义(χ^(2)=4.636,P<0.05)。结论实施个体化医学营养治疗可显著改善GDM患者的产后OGTT结果,并降低产后糖代谢异常的发生率。Objective To evaluate the effect of individualized medical nutrition therapy on postpartum blood glucose metabolism in patients with gestational diabetes mellitus(GDM).Methods 214 GDM patients who received routine antenatal examination and gave birth in Beiliu People's Hospital of Guangxi from June 2020 to June 2022 were randomly selected and divided into two groups.110 cases were assigned to the observation group receiving individualized medical nutrition therapy(IMNT)and 104 cases to the control group receiving routine nutrition guidance for gestational diabetes.The results of the oral glucose tolerance test at 6 weeks postpartum were compared between the two groups.Results The levels of FPG,1 h and 2 h postprandial blood glucose in the observation group were significantly lower than those in the control group at 42 days postpartum,and the difference was statistically significant(P<0.05).The incidence of abnormal glucose metabolism in the observation group was 30.91%,which was significantly lower than that in the control group 45.20%,and the difference was statistically significant(χ2=4.636,P<0.05).Conclusion The implementation of individualized medical nutrition therapy can significantly improve the postpartum OGTT results of GDM patients and reduce the incidence of postpartum glucose metabolism abnormalities.
